摘　　要：目的在客观评价的基础上,进一步评估骨盆骨折尿道损伤患者对于尿道成形术的满意度。方法收集2019年1月1日-2020年6月30日因骨盆骨折尿道损伤来我院就诊的48例成年男性患者的临床资料。术后6个月用尿道手术相关的患者主观评价问卷进行随访。主观评价问卷主要包括下尿路症状、生活质量评分以及对手术是否满意。客观评价指标包括术后尿流率以及术后尿道镜检查。结果共纳入48例患者进行研究,平均年龄46岁,均由同一术者行经会阴尿道吻合术。患者主观评价结果中38例(79.2%)显示对手术结果很满意以及非常满意,客观评价结果示41例(85.4%)手术成功。与患者主观评价中对手术结果不满意相关的因素主要有尿道狭窄的长度、尿道手术史、术后尿流率的结果以及尿道镜检查的结果、持续的下尿路症状等(P<0.05)。结论骨盆骨折尿道损伤患者尿道吻合术后患者的主观评价结果与客观评价结果基本一致,患者对手术的满意度总体较高,但仍有一定复发率。Objective To evaluate objective surgery success and subjective patient-reported outcome measure(PROM)in patients with pelvic fracture urethral injury(PFUI).Methods The clinical data of male patients over 18 years old who came to our hospital for PFUI andunderwent urethroplasty during Jan.1,2019 and Jun.30,2020 were collected.Patient-reported Outcome Measure for Urethral Stricture Surgery(USS-PROM)was used to subjectively follow up patients’lower urinary tract symptom(LUTS),quality of life scores and satisfaction with the surgical results 6 months after surgery.Postoperative objective evaluation indicators were also collected,including uroflowmetry and urethroscopy results.Results A total of 48 patients with an averaged age of 46 years were included in the study.They all underwent urethroplasty by the same surgeon.Of all patients,38(79.2%)indicated that they were satisfied or very satisfied with the surgical results.Objective evaluation showed that 41 operations were successful.The main factors related to dissatisfaction after urethroplasty were the length of urethral stricture,history of urethral surgery,postoperative urinary flow rate,urethroscopyresults and persistent LUTS(P<0.05).Conclusion The subjective evaluation results of PFUI patients after urethroplasty were basically consistent with the objective evaluation results.The patients’satisfaction with the operation was generally high,but there was still a certain recurrence rate.
