摘　　要：目的探讨超声引导下肩周注射抗炎镇痛液配合外用麝香追风止痛膏治疗肩周炎患者的临床疗效。方法收集2020年1月至2020年12月新乡医学院第一附属医院疼痛科门诊诊治的肩周炎疼痛期的患者86例作为观察对象,进行回顾性分析。根据治疗方法不同将其分为联合治疗组与口服药物组,每组43例,联合治疗组采用超声引导下肩周注射抗炎镇痛液配合外用麝香追风止痛膏治疗,抗炎镇痛液的配制为2%利多卡因5 ml+曲安奈德10 mg,以0.9%生理盐水稀释至20 ml,每周注射1次,共治疗3次,每次治疗后次日肩关节局部外用麝香追风止痛膏,每日1贴,共10贴,注射前1 d、注射后当日停用麝香追风止痛膏。口服药物组治疗采用美洛昔康7.5 mg口服,每日1次;替扎尼定片1 mg口服,每日3次,服用2周。观察治疗前、治疗后1个月时两组患者的疼痛数字等级评分(NRS)、匹兹堡睡眠质量指数(PSQI)、肩关节活动度,比较两组患者的各项观察指标及疗效。结果两组患者在性别、年龄、病程与治疗前NRS评分方面差异均无统计学意义(P均>0.05),具有可比性。治疗后1个月时两组NRS及PSQI评分均低于治疗前,差异均有统计学意义(P均<0.05);与口服药物组比较,联合治疗组NRS评分、PSQI评分均低于口服药物组,肩关节功能改善更好,差异均有统计学意义(P均<0.05),联合治疗组总有效率高于口服药物组,差异有统计学意义(P<0.05)。结论超声引导下肩周注射抗炎镇痛液配合外用麝香追风止痛膏治疗肩周炎,可有效缓解疼痛,提高睡眠质量,改善肩关节功能,疗效较口服药物显著。Objective To explore the clinical efficacy of ultrasound-guided peri-shoulder injection of anti-inflammatory analgesic solution and external application of musk chase analgesic paste in the treatment of shoulder periarthritis.Methods Between January 2020 and December 2020,86 patients with periarthritis in The Department of Pain Clinic,The First Affiliated Hospital of Xinxiang Medical College,were enrolled and divided into combined treatment group and oral drug group,43 cases in each group.The patients were treated by ultrasound-guided peri-shoulders injection of anti-inflammatory analgesic solution and topical musk chase analgesic paste in the combined treatment group.The anti-inflammatory analgesic fluid was prepared as 2%lidocaine 5 ml+triamcinolone acetonide 10 mg,with 0.9%normal saline diluted to 20 ml,and injected once a week,a total of 3 times;the next day after each injection,musk chase analgesic paste was applied topically,1 paste a day,a total of 10 paste.The patients in the oral medication group were treated with meloxicam 7.5 mg orally,once a day;tizanidine tablet 1 mg orally,3 times a day,for 2 weeks.The numerical rating scale(NRS),Pittsburgh sleep quality index(PSQI)and range of shoulder motion were compared in the two groups before the treatment and 1 month after the treatment.Results There were no significant differences in gender,age,course of disease and NRS before the treatment between the two groups(all P>0.05).One month after the treatment,the NRS and PSQI were lower in both groups than those before the treatment(all P<0.05).NRS and PSQI were lower and the shoulder joint function improved better in the combined treatment group than those in the oral medication group(all P<0.05).The total effective rate was higher in the combined treatment group than that in the oral medication group(P<0.05).Conclusion Ultrasound-guided peri-shoulder injection of anti-inflammatory analgesic solution combined with topical musk chase analgesic paste can effectively relieve pain,improve sleep quality and improve t
