精细化护理在急性下消化道出血患者中的应用

摘　　要：目的探讨在急性下消化道出血患者的护理中应用精细化护理的临床效果。方法选取2018年2月至2022年2月就诊于天津医科大学第二医院胃肠外科的102例下消化道出血患者为研究对象,通过随机数表法将上述研究对象分为对照组及观察组,其中观察组52例,对照组50例。对照组患者给予常规护理,观察组患者给予精细化护理,对比两组患者的护理满意度评分、护理前后焦虑自评量表(self-rating anxiety scale,SAS)评分、护理前后抑郁自评量表(self-rating depression scale,SDS)评分及出血控制情况。结果在护理满意度方面:观察组与对照组两组比较差异有统计学意义(P<0.05);经护理后,两组患者SAS、SDS评分与护理前相比均有所下降;与对照组相比,观察组SAS、SDS评分下降更多,差异有统计学意义(P<0.05)。观察组患者中,有4例患者发生再出血;对照组中,有11例患者发生再出血,两组差异有统计学意义(P<0.05)。结论对急性下消化道出血患者实行精细化护理能极大地提高护理满意度,改善患者抑郁状态,有利于减少消化道出血的发生。Objective To study the clinical effects of the application of refined nursing care to the care of patients with acute lower gastrointestinal bleeding.Methods 102 patients with lower gastrointestinal bleeding who visited the department of gastrointestinal surgery of The Second Hospital of Tianjin Medical University from February 2018 to February 2022 were selected as study subjects,and the above study subjects were divided into control group and observation group by random number table method,including 52 observation and 50 control subjects.Patients in the control group were given routine care,patients in the observation group were given refined care,and the nursing satisfaction scores,self-rating anxiety scale(SAS)scores before and after care,self-rating depression scale(SDS)scores before and after care,and bleeding control were compared between the two groups.Results In terms of nursing satisfaction:the difference between the observation group and the control group was statistically significant(P<0.05).After nursing care,the SAS and SDS scores of patients in both groups decreased compared with those before nursing care,and the SAS and SDS scores of the observation group decreased more compared with those of the control group,the difference was statistically significant(P<0.05).4 patients had rebleeding in the observation group,while 11 patients had rebleeding in the control group,and the difference between the two groups was statistically significant(P<0.05).Conclusion The implementation of refined nursing care for patients with acute lower gastrointestinal bleeding can greatly improve nursing satisfaction,improve the depression status of patients,and it is beneficial for the control of gastrointestinal bleeding.
