HPV感染检测在基层妇女宫颈病变筛查中的应用分析

摘　　要：目的分析将人乳头瘤状病毒(HPV)感染检测应用于基层妇女宫颈病变筛查中的效果。方法选取于本院接受宫颈病变筛查的妇女100例为研究对象,对其进行薄层液基细胞学(TCT)与高危型HPV-DNA检测,检测结果阳性者接受阴道镜下多点宫颈组织活检,以此评价两种筛查方法单一使用及联合使用的应用价值并分析HPV感染情况。结果联合检测的敏感度、特异度及阴性预测值均明显优于单一检测,差异有统计学意义(P<0.05);HPV-DNA检测的敏感度、阳性预测值及阴性预测值均略高于TCT检测,但二者比较,差异无统计学意义(P>0.05)。HPV-DNA阳性患者中,30岁以上妇女的感染率为68.29%,明显高于30岁以下的31.71%,差异有统计学意义(P<0.05)。结论HPV-DNA阳性高发于宫颈癌前病变妇女,联合HPV与TCT进一步检测宫颈病变性质,其筛查意义与检测价值对30岁以上妇女更加显著。Objective To analyze the effect of human papillomavirus(HPV)infection detection in the of cervical lesions screening in grass-roots women.Methods A total of 100 women who were screened for cervical lesions in our hospital were selected as the research objects.Thinprep cytologic test(TCT)and high-risk HPV-DNA were tested for them.Those who tested positive underwent colposcopy for multiple cervical biopsies.The application value of single or combined use of the two screening methods was evaluateand and the status of HPV infection was analyzed.Results The sensitivity specificity and negative predictive value of combined detection were significantly better than those of single detection(P<0.05);the sensitivity,positive predictive value and negative predictive value of HPV-DNA test were slightly higher than those of TCT test,but there were no significant differences between them(P>0.05).Among HPV-DNA positive patients,the infection rate of women over 30 years old was 68.29%,which was significantly higher than 31.71%of women under 30 years old,and the difference was statistically significant(P<0.05).Conclusion HPV-DNA positivity is high in women with cervical precancerous lesions.The screening significance and detection value of HPV combined with TCT for further detection of cervical lesions are more significant in women over 30 years old.
