HPV在宫颈炎中的检测意义分析

摘　　要：目的分析人乳头瘤病毒(HPV)在宫颈炎中的检测意义。方法回顾性分析于本院行HPV分型检测的80例患者的临床资料,比较患者病理检查及HPV检测的结果,分析HPV分型在宫颈炎中的分布情况及HPV感染的影响因素。结果HPV检测的敏感度为57.58%(19/33),特异度为74.47%(35/47),阳性预测值为0.613(19/31),阴性预测值为0.714(35/49)o宫颈炎HPV感染以HPV52(52.6%)、HPV16(26.3%)、HPV18(10.5%)、HPV53(5.3%)、HPV58(5.3%)为主。存在生殖道炎症的人群为HPV感染的高发群体,其感染HPV的概率为64.52%,高于不存在生殖道炎症人群的24.49%,差异有统计学意义(P<0.05)。不同年龄人群间的HPV感染率比较,差异无统计学意义(P>0.05)o结论HPV分型检测对宫颈炎有较高的诊断敏感度及特异度,HPV52、16、18、53、58为宫颈炎主要感染的HPV亚型,存在生殖道炎症为HPV感染的主要原因。Objective To analyze the detection significance of human papilloma virus(HPV)in cervicitis.Methods The clinical data of 80 patients with HPV typing in our hospital were retrospectively analyzed,and the results of pathologic examination and HPV testing were compared,the distribution of HPV typing in cervicitis and the influencing factors of HPV infection were analyzed.Results The sensitivity of HPV detection was 57.58%(19/33),the specificity was 74.47%(35/47),the positive predictive value was 0.613(19/31),and the negative predictive value was 0.714(35/49).HPV infection of cervicitis was mainly caused by HPV 52(52.6%),HPV 16(26.3%),HPV18(10.5%),HPV 53(5.3%)and HPV 58(5.3%).The population with genital tract inflammation was a high incidence group of HPV infection,and the probability of HPV infection was 64.52%,which was higher than 24.49%in the population without genital tract inflammation,and the difference was statistically significant(P<0.05).There were no significant differences in HPV infection rates among different age groups(P>0.05).Conclusion HPV typing test has high sensitivity and specificity in the diagnosis of cervicitis.HPV52,16,1&53,58 are the main HPV subtypes infected with cervicitis,and genital tract inflammation is the main cause of HPV infection.
