摘  要:为预测宫颈癌容积旋转调强(VMAT)放疗膀胱和直肠剂量,建立相关模型指导临床。以44例宫颈癌VMAT放疗为例,提取靶区和危及器官膀胱和直肠的解剖空间位置关系,以宫颈癌靶区体积、靶区处方剂量、膀胱体积、直肠体积以及靶区和膀胱、直肠的空间位置特征为输入量,基于剂量−体积直方图(DVH)数据和Python工具包,以膀胱、直肠的V_(40)和D_(33)作为输出量,经主成分分析建立回归方程模型预测膀胱和直肠的剂量。实际值与预测值相比,膀胱和直肠的结果都较为精确,其中,直肠V_(40)的相对误差范围在1.40%~6.89%,直肠D_(33)的相对误差范围在2.12%~6.81%;膀胱V_(40)的相对误差范围在2.47%~6.67%,膀胱D_(33)的相对误差范围在0.38%~4.28%,实际值与预测值的差异均无统计学意义(p>0.05)。该模型可以有效预测宫颈癌调强放疗计划膀胱直肠受量,对临床放疗工作有指导作用。To predict rectal and bladder radiation doses during volumetric modulated arc therapy(VMAT)for cervical cancer,a relevant model was established to guide clinical practice.Forty-four cases of cervical cancer treated using VMAT radiotherapy were retrospectively analyzed.The positional relationships of the anatomical spaces between the cervical cancer target volume and the organs at risk(bladder and rectum)were evaluated.Volume and spatial location characteristics of the target volume for the bladder and rectum were inserted as input data in a dose-volume histogram and Python toolkit for statistical analysis.The output data were the bladder and rectal V_(40) and D_(33).A mathematical model was developed to predict the bladder and rectal doses.When the actual and predicted values were compared,they were quite similar for both the bladder and rectal doses.The relative error ranges of rectal V_(40) and D_(33) were between 1.40%and 6.89%,and between 2.12%and 6.81%,respectively.Likewise,the relative error ranges of bladder V_(40) and D_(33) were between 2.47%and 6.67%,and between 0.38%and 4.28%,respectively.There were no significant differences between the actual and predicted values(p>0.05).This model can effectively predict the bladder and rectal doses needed during intensity-modulated radiotherapy for cervical cancer and could serve as a guide for radiotherapy clinical practice.
