重度意识障碍患者发生便秘的相关因素分析

摘　　要：目的:分析重度意识障碍(DOC)患者发生便秘的危险因素,为预防或减少其发生提供临床依据。方法:以某三级甲等医院高压氧科2019年6月至2021年6月收治的重度DOC患者作为研究对象,根据是否发生便秘分为便秘组和非便秘组,对2组患者的临床资料进行回顾性分析,探讨其发生便秘的危险因素。结果:290例患者的便秘发生率为35.2%;单因素分析结果显示,非纤维型营养制剂、镇静剂、机械通气的应用及预防性使用泻药对重度DOC患者发生便秘有影响(P<0.05);多因素Logistic回归分析显示,应用镇静剂、机械通气是重度DOC患者发生便秘的危险因素(P<0.05),预防性使用泻药是其保护因素(P<0.05)。结论:重度DOC患者发生便秘受多种因素影响,在护理上需对多个环节加以关注和干预。Objective To find out the risk factors of constipation in patients with severe disorder of consciousness(DOC),so as to provide clinical evidence for preventing or reducing its occurrence.Methods The severe DOC patients admitted to the Department of Hyperbaric Oxygen of a grade A level-3 hospital from June 2019 to June 2021 were selected as research subjects,and they were divided into constipation group and non-constipation group according to whether constipation occurred.The clinical data of the two groups were retrospectively analyzed to study the risk factors of constipation.Results The incidence of constipation in 290 patients was 35.2%.Univariate analysis showed that non-fiber nutrition preparation,sedative use,mechanical ventilation,and preventive use of laxatives had affected the occurrence of constipation in patients with severe DOC(P<0.05).Multivariate Logistic regression analysis showed that sedative use and mechanical ventilation were the risk factors for the occurrence of constipation in severe DOC patients(P<0.05),and the preventive use of laxatives was a protective factor for that(P<0.05).Conclusion The occurrence of constipation in patients with severe DOC is affected by many factors,and thus attention should be paid and intervention should be taken in nursing.
