摘  要:目的探讨多种影像学方法评估电针肩部腧穴治疗中风后肩关节半脱位疗效的诊断效能。方法选取2020年6月~2021年1月黑龙江中医药大学附属第二医院中风后肩关节半脱位患者60例,分为实验组与对照组,各30例,对照组采取普通针刺疗法,实验组采用电针肩部腧穴治疗,治疗后肩关节X线肩峰下间隙(acromio-humeralinterval,AHI)测量联合磁共振成像肩关节情况评估,应用FugI-Meyer量表评定治疗前后上肢运动功能差异。结果实验组治疗后FugI-Meyer量表评分为(59.94±5.32)分,对照组为(40.98±5.11)分,差异具有统计学意义(P<0.05);治疗前后肩关节运动功能评分均有明显上升,差异有统计学意义(P<0.05);实验组患侧AHI值分别为(25.21±3.01)mm和(14.45±1.54)mm,对照组患侧AHI值分别为(25.11±2.98)mm和(11.34±1.33)mm,差异有统计学意义(P<0.05);1.5T磁共振扫描各项异常均明显改善,磁共振实验组治疗后总有效率为96.67%,对照组为60%,差异有统计学意义(P<0.05)。X线测量结果与FugI-Meyer评分呈中度相关性(r=0.45,P<0.05);磁共振观察结果与FugI-Meyer评分呈高度相关性(r=0.78,P<0.05)。结论对于电针肩部腧穴治疗中风后肩关节半脱位患者应用X线与磁共振联合评估对其肩关节评价更为有效,值得推广应用。Objective To evaluate the therapeutic effect of electroacupuncture at shoulder acupoints on shoulder subluxation after stroke with various imaging methods.Methods From June 2020 to January 2021,sixty cases of shoulder subluxation after stroke in the Second Affiliated Hospital of Heilongjiang University of Traditional Chinese Medicine were selected,30 cases in the experimental group and 30 cases in the control group.The control group was treated with ordinary acupuncture,while the experimental group was treated with electroacupuncture at shoulder acupoints.After treatment,the shoulder joint X-ray acromio-humeralinterval(AHI)measurement combined with magnetic resonance imaging(MRI)shoulder joint evaluation was performed,and FugI-Meyer scale was used to evaluate the difference of upper limb motor function before and after treatment.Results The score of FugI-Meyer scale was(59.94±5.32)in the experimental group and(40.98±5.11)in the control group after treatment,the difference was statistically significant(P<0.05).The AHI values of the affected side in the experimental group were(25.21±3.01)mm and(14.45±1.54)mm,respectively,the AHI values of the control group were(25.11±2.98)mm and(11.34±1.33)mm,respectively,the difference was statistically significant(P<0.05);The abnormalities of 1.5 T MRI scanning were significantly improved,the total effective rate of the experimental group was 96.67%,and that of the control group was 60%,the difference was statistically significant(P<0.05).There was a moderate correlation between X-ray measurement results and FugI-Meyer score(r=0.45,P<0.05),and a high correlation between MRI observation results and FugI-Meyer score(r=0.78,P<0.05).Conclusion For patients with shoulder subluxation after stroke treated by electroacupuncture at shoulder acupoints,the combined evaluation of X-ray and MRI is more effective,which is worthy of popularization and application.
