荜铃胃痛颗粒联合幽门螺杆菌根除四联疗法治疗胃食管反流病合并幽门螺杆菌感染气滞血瘀证的疗效观察

摘　　要：目的观察荜铃胃痛颗粒联合幽门螺杆菌(Hp)根除四联疗法治疗胃食管反流病(GERD)合并Hp感染气滞血瘀证的临床疗效。方法将GERD合并Hp感染气滞血瘀证患者按照随机数字表法分为2组,对照组40例采取Hp根除四联疗法治疗,治疗组40例在对照组的基础上联合荜铃胃痛颗粒治疗。2组均治疗8周。比较2组治疗前后胃食管反流病自测表(Gerd Q)评分、中医证候评分、内镜下分级情况,并统计2组临床疗效及Hp根除率。结果治疗组总有效率92.50%(37/40),对照组总有效率75.00%(30/40),治疗组疗效优于对照组(P<0.05)。2组治疗后Gerd Q评分均较本组治疗前降低(P<0.05),且治疗组治疗后低于对照组(P<0.05)。2组治疗后中医证候各项评分及总分均较本组治疗前降低(P<0.05),且治疗组治疗后均低于对照组(P<0.05)。2组治疗后Savery-Miller分级均较本组治疗前改善(P<0.05),且治疗组改善优于对照组(P<0.05)。治疗组Hp根除率95.00%(38/40),对照组Hp根除率77.50%(31/40),治疗组Hp根除率高于对照组(P<0.05)。结论荜铃胃痛颗粒联合Hp根除四联疗法治疗,可显著改善GERD合并Hp感染患者的临床症状,降低食管黏膜损伤程度,提高Hp根除率及临床疗效。Objective To observe the clinical effect of Biling Weitong Granule combined with Helicobacter pylori(Hp)eradication-based quadruple therapy for gastroesophageal reflux disease(GERD)of Qi-blood stagnation syndrome.Methods The GERD patients(Qi-blood stagnation syndrome)were randomly assigned in a 1:1 ratio to receive either quadruple therapy or Biling Weitong Granule+quadruple therapy for 8 weeks.The key observation was the Gastroesophageal Reflux Disease Questionnaire(Gerd Q),traditional Chinese medicine(TCM)syndrome score,endoscopic classification;the curative effect and eradication rate were assessed.Results The overall effective rate was significantly higher in the treatment group than that in the control group(92.50%[37/40]vs 75.00%[30/40];(P<0.05),respectively).After treatment,Gerd Q scores in groups were markedly decreased(P<0.05),the treatment group was more common(P<0.05);TCM symptom scores in groups were obviously decreased(P<0.05),which decreased in the treatment group compared with the control group(P<0.05);the Savery-Miller classification in groups was significantly improved(P<0.05),improvement of which in the treatment group was significantly better(P<0.05).The Hp eradication rate in the treatment group was significantly better than that in the control group(95.00%[38/40]vs 77.50%[31/40],[P<0.05],respectively).Conclusion For GERD patients,Biling Weitong Granule combined with quadruple therapy for Hp eradication can significantly improve clinical symptoms,reduce esophageal mucosa injury,improve Hp eradication rate and clinical efficacy.
