调肝防早方干预肝郁化火型女童特发性性早熟的临床疗效研究

摘　　要：目的:研究调肝防早方干预特发性中枢性性早熟(ICPP)女童(肝郁化火证)的临床疗效。方法:门诊收集2020年10月至2021年6月60例ICPP女性患儿,并将其随机纳入丹栀逍遥丸组与调肝防早方组,每组30名患儿。分别通过给予丹栀逍遥丸、调肝防早方干预,观察其干预前后乳房Tanner分期、促黄体生成素(LH)、促卵泡激素(FSH)、血清雌激素(E_(2))、类胰岛素一号增长因子(IGF-1)、食欲素(orexin)水平的变化、子宫、卵巢体积、最大卵泡直径及中医证候积分。结果:调肝防早方组乳房Tanner分期较干预前变化显著(P<0.05),丹栀逍遥丸组与前比较差异无显著性(P>0.05)。2组干预后血清LH、FSH、E_(2)水平均较前降低(P<0.05),且调肝防早方组降低更显著(P<0.05)。2组干预后血清IGF-1水平均较干预前降低(P<0.05),orexin水平均较前升高,且调肝防早方组变化更明显(P<0.05),表明调肝防早方可有效调节IGF-1、orexin水平。2组干预后子宫体积、最大卵泡直径均较前明显降低(P<0.05),且以调肝防早方组为著(P<0.05)。调肝防早方组干预后卵巢体积较前明显缩小(P<0.05),丹栀逍遥丸组则较前比较无明显差异(P>0.05)。2组患者干预后中医证候总积分均较前降低(P<0.05),且调肝防早方组降低更明显(P<0.05)。结论:调肝防早方干预ICPP女童(肝郁化火证)可减缓乳房发育,降低激素及IGF-1水平,减小子宫体积、卵巢体积、最大卵泡直径、减轻中医证候。中医治疗ICPP为临床提供了新思路、新方法,且疗效确切。Objective:To study the clinical effect of Tiaogan Fangzao Formula(formula for regulating liver funciton and preventing precocity)on idiopathic central precocious puberty(ICPP)in girls with the pattern of liver depression transforming into fire.Methods:60 female children with ICPP from October 2020 to June 2021 were collected in the outpatient department and randomly included into the Danzhi Xiaoyao Pill group and the Tiaogan Fangzao Formula group,with 30 children in each group.Tanner staging of breast,luteinizing hormone(LH),follicle stimulating hormone(FSH),serum estrogen(E_(2)),insulin-like growth factor 1(IGF-1),orexin level changes,uterine and ovarian volume,maximum follicle diameter and TCM syndrome score were observed before and after the intervention of Danzhi Xiaoyao Pill and Tiaogan Fangzao Formula.Results:The Tanner staging of breast in Tiaogan Fangzao Formula group changed significantly compared with that before intervention(P<0.05)while there was no significant changes in Danzhi Xiaoyao Pill group(P>0.05);The levels of serum LH,FSH and E_(2) in both groups were lower than that before intervention(P<0.05),more significantly in the Tiaogan Fangzao Formula group(P<0.05);After intervention,the serum IGF-1 level in both groups was lowered(P<0.05),orexin increased,and the chagnes were more significant in the Tiaogan Fangzao Formula group(P<0.05),indicating the formula could effectivly regulate IGF-1 and orexin levels.After intervention,the uterine volume and the largest follicle diameter of the two groups were significantly reduced(P<0.05),more significantly in the Tiaogan Fangzao Formula group(P<0.05);After intervention,the ovarian volume in the Tiaogan Fangzao Formula group decreased significantly(P<0.05),while the Danzhi Xiaoyao Pill group had no significant difference(P>0.05).After intervention,the total score of TCM syndromes in both groups decreased(P<0.05).Conclusion:Tiaogan Fangzao Formula can slow down the breast development,reduce the hormone and IGF-1 levels,reduce the uterine volume,ovarian volu
