自体富血小板血浆联合脉血康胶囊治疗糖尿病足患者的效果

摘　　要：目的:观察自体富血小板血浆(PRP)联合脉血康胶囊治疗糖尿病足患者的效果。方法:选取2018年11月至2020年7月该院收治的90例糖尿病足患者进行前瞻性研究,按照随机数字表法分对照组和观察组各45例。对照组给予脉血康胶囊治疗,观察组在对照组基础上联合自体PRP治疗,比较两组临床疗效,治疗前后创面面积,血小板源性生长因子(PDGF)、转化生长因子(TGF-β1)水平,预后情况、下肢动脉功能指标[踝肱指数(ABI)、趾肱指数(TBI)、震动感觉阈值(VPT)]水平及不良反应发生率。结果:观察组治疗总有效率为95.56%,高于对照组的82.22%,差异有统计学意义(P<0.05);治疗后,观察组创面面积小于对照组,PDGF、TGF-β1水平均高于对照组,差异有统计学意义(P<0.05);观察组足部腐肉脱净时间、边缘新鲜肉芽长出时间、基底新鲜肉芽长出时间均短于对照组,差异有统计学意义(P<0.05);观察组ABI、TBI水平均高于对照组,VPT水平低于对照组,差异有统计学意义(P<0.05);两组均未发生明显不良反应。结论:自体PRP联合脉血康胶囊治疗糖尿病足患者,可提高临床疗效,加快创面愈合,提高PDGF和TGF-β1水平,降低VPT水平,改善下肢动脉血液循环,其效果优于单纯血脉康胶囊治疗。Objective:To observe effects of autologous platelet-rich plasma(PRP)combined with Maixuekang capsules in treatment of diabetic foot.Methods:A prospective study was performed on 90 diabetic foot patients admitted to this hospital from November 2018 to July 2020,and they were divided into control group and observation group according to the random number table method,45 cases in each.The control group was treated with Maixuekang capsules,while the observation group was treated with autologous PRP on the basis of that of the control group.The clinical efficacy,the wound area,the levels of platelet-derived growth factor(PDGF)and transforming growth factor-β1(TGF-β1),the prognosis,the lower extremity arterial function index[ankle-brachial index(ABI),toe-brachial index(TBI),vibration perception threshold(VPT)]levels,and the incidence of adverse reactions were compared between the two groups before and after the treatment.Results:The total effective rate of treatment in the observation group was 95.56%,which was higher than 82.22%in the control group,and the difference was statistically significant(P<0.05).After the treatment,the wound area in the observation group was smaller than that in the control group;the levels of PDGF and TGF-β1 were higher than those in the control group;and the differences were statistically significant(P<0.05).The removal time of foot slough,the growth time of edge fresh granulation,and the growth time of fresh granulation at the base of the observation group were all shorter than those of the control group,and the differences were statistically significant(P<0.05).The levels of ABI and TBI in the observation group were higher than those in the control group,the level of VPT was lower than that in the control group,and the differences were statistically significant(P<0.05).However,there were no obvious adverse reactions in the two groups.Conclusions:Autologous PRP combined with Maixuekang capsule in the treatment of diabetic foot patients can improve the clinical efficacy,speed up wound he
