摘　　要：终末期血友病性骨关节病患者在合理的凝血因子替代治疗下,可以相对安全地接受髋膝关节置换手术。围手术期凝血因子替代剂量、持续时间,手术适应证的掌握,手术规划,围手术期功能训练与康复等直接影响手术的安全性与临床效果。为了进一步规范血友病性骨关节病髋膝关节置换围手术期管理、提高疗效,中华医学会骨科学分会关节外科学组和中国血友病协作组汇集国内骨科学、血液病学、康复医学、检验医学、临床药学和循证医学等领域专家,经过多轮讨论,并在参考国内外相关文献证据的基础上编写了本指南,供临床医师参考。End stage hemophilic arthropathy can be treated by hip and/or knee arthroplasty with the replacement therapy of coagulation factors. The dose and duration of coagulation factors in perioperative period, surgical indications, surgical planning,perioperative functional training, and rehabilitation are closely related to surgical safety and clinical outcomes. To standardize hip and knee arthroplasty for hemophilic arthropathy and improve therapeutic effect, after panel discussions, multidisciplinary experts from orthopedics, hematology, physical rehabilitation, laboratory medicine, clinical pharmacy and evidence-based medicine compiled this guideline in reference to clinical evidences.
