中医定向透药配合穴位敷贴对ICU机械通气患者胃肠功能障碍的疗效观察

摘　　要：目的:观察中医定向透药配合穴位敷贴对ICU机械通气患者胃肠功能障碍(GID)的影响。方法:选取本院2019年2月-2020年8月收治的ICU机械通气伴GID患者120例,按随机数字表法分为3组,各40例。对照组行常规护理干预,观察A组行中医定向透药干预,观察B组行中医定向透药配合穴位敷贴干预,3组均持续干预1个月。比较3组营养指标、胃肠道不耐受情况发生率和胃肠功能恢复情况。结果:观察B组、观察A组干预后白蛋白、氮平衡、总蛋白高于对照组,且观察B组高于观察A组,差异有统计学意义(P<0.05);观察B组、观察A组胃肠道不耐受情况发生率低于对照组,差异有统计学意义(P<0.05);观察B组、观察A组肛门排气、肠鸣音恢复及恢复进食时间均短于对照组,且观察B组短于观察A组,差异有统计学意义(P<0.05)。结论:中医定向透药配合穴位敷贴可改善ICU机械通气患者营养状况,促进胃肠功能恢复,减少胃肠道不耐受情况发生。Objective:To explore the effect of directional drug penetration of traditional Chinese medicine combined with acupoint application on gastrointestinal dysfunction(GID)of mechanically ventilated patients in ICU.Methods:A total of 120 patients with mechanical ventilation in ICU with GID who were admitted to our hospital from February 2019 to August 2020 were selected and divided into 3 groups according to the random number table method,40 cases in each group.The control group underwent routine nursing intervention,observation group A underwent TCM directional drug penetration intervention,and observation group B underwent TCM directional drug penetration intervention combined with acupoint application.The nutritional indicators,the incidence of gastrointestinal intolerance and the recovery of gastrointestinal function of the three groups were compared.Results:The albumin,nitrogen balance and total protein of the observation group B and A were higher than those of the control group after intervention,and the observation group B was higher than the observation group A,there was a statistical difference(P<0.05);The incidence of gastrointestinal intolerance in the observation group A and group B was lower than that in the control group,and there was a statistical difference(P<0.05);Anal exhaustion,bowel sound recovery and the recovery time of eating of observation group B and group A were shorter than those in control group,and observation group B is shorter than observation group A,there is a statistical difference(P<0.05).Conclusion:TCM directional drug penetration combined with acupoint application can improve the nutritional status of ICU patients with mechanical ventilation,promote the recovery of gastrointestinal function,and reduce the occurrence of gastrointestinal intolerance.
