匍行性回状红斑一例

摘　　要：患者,女,47岁。全身环状红斑伴下肢疼痛4天。皮肤科查体:四肢及躯干皮肤见泛发环状、回状红斑,环状边缘隆起,压之不褪色,局部见环状色素沉着。皮肤组织病理示:网蓝状角化,表皮大致正常,真皮浅层血管附属器周围见大量红细胞外溢,中等量中性粒细胞浸润,可见核尘,部分血管壁可见血管纤维素样坏死。诊断:匍行性回状红斑。A 47-year-old female presented with systemic annular erythema and lower limb pain for 4 days.Physical examination showed lesions consist of slightly elevated erythema on the limbs and trunk,with ring pigmentation.Histologic examination showed the reticular blue keratosis,a large number of red blood cells overflowed around the vascular appendages in the superficial dermis,a moderate amount of neutrophils infiltrated,nuclear dust was visible,and vascular cellulose like necrosis was visible in some vascular walls.The diagnosis of erythema gyratum repens was made.
