摘  要:结核病(TB)传播力强,危害性大,目前仍是全球主要的传染性疾病。由于采取了积极有效的措施,TB在世界范围内的流行得到了一定的控制,但随着人类免疫缺陷病毒(HIV)感染的流行,TB又有死灰复燃的趋势。TB是HIV感染者中常见的机会性感染和死亡的主要原因之一,HIV与TB双重感染(HIV/TB)发病机制错综复杂,现有研究表明结核分枝杆菌可通过诱导单核细胞趋化蛋白-1表达增强HIV-1在机体的复制转录,使结核感染区域的HIV转录量升高,加速病毒增殖,使病情恶化。此外,CD4+T细胞、Th17/Treg免疫轴、肺泡巨噬细胞、NLRP3炎症小体、组织蛋白酶均在HIV/TB共感染机制起着重要作用。本文旨在阐明HIV/TB双重感染的多种复杂机制。Tuberculosis(TB)is a highly contagious and harmful disease,and remains the world′s leading infectious disease.Due to the positive and effective measures taken,the tendency of TB was once controlled in the world,but the tendency resurged with the prevalence of human immunodeficiency virus(HIV)infection.TB is the main cause of opportunistic infection and death in HIV-infected people,the dual pathogenesis of HIV/TB co-infection is perplexing.The current research indicates that Mycobacterium tuberculosis can enhance the replication and transcription of HIV-1 in the body by inducing the expression of monocyte chemoattractant protein-1,which increases the HIV transcription level in the tuberculosis-infected area,then accelerates virus multiplication and aggravates the disease.In addition,CD4+T cells,Th17/Treg immune axis,alveolar macrophages,NLRP3 inflammatory bodies,and cathepsins all play an important role in the mechanism of HIV/TB co-infection.This article aims to clarify the multiple complex mechanisms of HIV/TB co-infection.
