摘 要:目的探讨汉族孕妇发生RhE抗原同种免疫风险与HLA-DQA1等位基因多态性的相关性。方法对汉族孕妇进行不规则抗体筛查、抗-E抗体鉴定及RhE抗原鉴定;根据不规则抗体筛查和RhE抗原鉴定结果将RhE抗原阴性,产生抗-E抗体的汉族孕妇样本(排除输血史等)纳为试验组;RhE抗原阴性、不规则抗体阴性的汉族孕妇纳为对照组;采用PCR-SBT方法,将试验组和对照组样本的HLA-DQA1基因进行高分辨分型,获得两组人群HLA-DQA1基因的分布特征和多态性特点;使用Arlequin 3.5软件的EM算法计算HLA-DQA1等位基因和表型频率;使用卡方或Fisher精确检验比较HLA-DQA1等位基因、表型频率。结果在28412例孕妇中,筛选出67例为试验组、90例为对照组;产生抗-E抗体试验组HLA-DQA1*03:02表型频率明显高于对照组(OR 3.816;95%CI 1.923~7.569;Pc=0.001335)。结论汉族孕妇人群中,HLA-DQA1基因的多态性与RhE抗原同种免疫风险具有相关性,HLA-DQA1*03:02表型可能是RhE抗原同种免疫的一种易感表型。Objective To study genetic polymorphism of HLA-DQA1 associated with RhE alloimmunization in Han Chinese pregnant woman.Methods Irregular antibody screening and identification of anti-E antibody and RhE antigen were performed.Samples of Han Chinese pregnant women who were RhE antigen negative and produced anti-E antibodies(excluding blood transfusion history, etc.)were included in experimental group.RhE antigen negative and irregular blood group antibodies negative in Han Chinese pregnant women were included in control group.These samples were subjected to high-resolution genotyping of HLA-DQA1 genes using PCR-SBT,haplotypic frequencies were calculated via an expectation maximization(EM)algorithm using Arlequin version 3.5 software, HLA-DQA1 allele and genotype frequencies were compared by using Pearson’s chi-square test or Fisher’s exact test.Results 28412 cases pregnant women were screened for irregular blood group antibodies, 67 cases with anti-E alloantibodies were enrolled, and samples from 90 cases irregular blood group antibodies negative in Han Chinese pregnant women were used as controls. Phenotypic frequency of HLA-DQA103: 02 were overrepresented in E-alloimmunized patient group( OR3. 816,95% CI 1. 923 ~ 7. 569,Pc = 0. 001335). Conclusion HLA-DQA1 genes would be associated with Rh E alloimmunization in Han Chinese pregnant woman with HLA-DQA1*03: 02 as a risk allele.
