肺超声与胸部CT在成人社区获得性肺炎中诊断价值的比较分析

摘　　要：目的探讨肺超声(LUS)在成人社区获得性肺炎(CAP)诊断中的价值,比较其与胸部X线(CRX)、电子计算机断层扫描(CT)的影像学差异。方法选择2020年3月至2020年12月在我院初诊为CAP的成年患者100例,比较其LUS、CT、CXR间的影像学表现(48 h内),并以CT为“金标准”,通过灵敏度、特异度、阳性预测值及阴性预测值评估LUS对成人CAP的诊断价值。结果100例患者经CT诊断为CAP的69例(69.0%),LUS诊断CAP(灵敏度91.3%、特异度77.4%、阳性预测值90.0%,阴性预测80.0%)比CXR(灵敏度73.9%,特异度80.6%,阳性预测值89.5%,阴性预测值58.1%)有更好的诊断效果。与CT一致性评价Kappa=0.693(P<0.001),结论LUS是诊断成人CAP的可靠工具,其灵敏度、特异度、阳性预测值和阴性预测值均较好,不弱于CXR,与CT诊断结果具有一致性。Objective To explore values of Lung Ultrasound(LUS)in the diagnosis of adult community-acquired pneumonia(CAP),and to compare its imaging differences with chest X-ray(CRX)and computed tomography(CT).Methods From March 2020 to December 2020,100 adult patients with CAP initially diagnosed in our Hospital were selected.Imaging manifestations(within 48 hours)among LUS,CT and CXR were compared.Taking CT as"Gold Standard",diagnostic value of LUS in adult cap was evaluated by L sensitivity,specificity,positive predictive value and negative predictive value.Results Among 100 patients,69(69%)were diagnosed as cap by CT.The sensitivity,specificity,positive predictive and negative predictive value of LUS in the diagnosis of cap were 91.3%,77.4%,90.0%and 80.0%,respectively.Compared with CXR(sensitivity 73.9%,specificity 80.6%,positive predictive value 89.5%,negative predictive value 58.1%)had better diagnostic effect.With consistency evaluation CT,Kappa=0.693(P<0.001).Conclusion LUS would be a reliable tool for the diagnosis of adult cap.Its sensitivity,specificity,positive predictive value and negative predictive value could be better than CXR,which is consistent with the diagnostic results of CT.
