摘　　要：目的探讨二陈汤合三子养亲汤加减治疗慢性阻塞性肺疾病痰湿壅肺证对胸肺顺应性及气体交换参数的影响。方法选取2020年1月至2022年1月在九江市第三人民医院就诊的80例慢性阻塞性肺疾病痰湿壅肺证患者作为研究对象,采用随机数字表法将患者分为对照组与观察组,每组各40例。对照组患者采取西医常规对症治疗,观察组患者在对照组的基础上应用二陈汤合三子养亲汤加减治疗。比较两组患者的症状缓解时间、血清炎症因子指标、胸肺顺应性指标、气体交换参数、生活质量评分、不良反应发生率。结果观察组患者的各项症状缓解时间均短于对照组,差异有统计学意义(P<0.05)。观察组患者治疗后的血清C反应蛋白、降钙素原、白细胞介素-6、肿瘤坏死因子-α水平均低于对照组,差异有统计学意义(P<0.05)。观察组治疗后的胸肺总顺应性、胸廓顺应性、肺顺应性、1秒用力呼吸容积、用力呼吸容积/肺活量比值、生活质量评分均高于对照组,差异有统计学意义(P<0.05)。两组患者的不良反应总发生率比较,差异无统计学意义(P>0.05)。结论二陈汤合三子养亲汤加减治疗对慢性阻塞性肺疾病痰湿壅肺证患者的症状缓解作用良好,还可减轻患者机体内炎症反应,改善其胸肺顺应性、肺换气功能,有利于提升生活质量,且不良反应未增加,用药安全性良好。Objective To explore the effect of Erchen Decoction and Sanzi Yangqin Decoction on thoracic and pulmonary compliance and gas exchange parameters in the treatment of phlegm dampness blocking lung syndrome of chronic obstructive pulmonary disease.Methods A total of 80 patients with phlegm dampness blocking lung syndrome of chronic obstructive pulmonary disease treated in Jiujiang Third People′s Hospital from January 2020 to January 2022 were selected as the research objects.The patients were divided into control group and observation group by random number table method,with 40 cases in each group.The patients in the control group were treated with conventional symptomatic Western medicine,and the patients in the observation group were treated with Erchen Decoction and Sanzi Yangqin Decoction on the basis of the control group.The symptom relief time,serum inflammatory factors,thoracopulmonary compliance indexes,gas exchange parameters,quality of life score,and incidence of adverse reactions were compared between the two groups.Results The remission time of all symptoms in the observation group was shorter than that in the control group,the difference was statistically significant(P<0.05).The levels of serum C-reactive protein,procalcitonin,interleukin-6 and tumor necrosis factor-αafter treatment in the observation group were lower than those in the control group,the differences were statistically significant(P<0.05).The total chest and lung compliance,thoracic compliance,lung compliance,forced breathing volume in one second,forced breathing volume/vital capacity ratio and quality of life score after treatment in the observation group were higher than those in the control group,the differences were statistically significant(P<0.05).There was no statistically significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion Erchen Decoction combined with Sanzi Yangqin Decoction can alleviate the symptoms of patients with phlegm dampness blocking lung syndrome of chronic ob
