静息角复位在鼻尖成形术中的应用研究

摘　　要：目的 探讨静息角复位在鼻尖成形术中的临床应用效果。方法 自2019年6月至2021年3月,重庆当代整形美容医院对34例求美者行综合隆鼻术,采用肋软骨搭建支架,通过下外侧软骨缝合技术行静息角复位,改善鼻尖形态。术后观察是否出现“夹捏”样畸形、鼻翼沟变浅等问题,并进行疗效评价。结果 所有求美者术后获随访6~18个月,平均10.5个月,鼻尖高度均获得提升,无“夹捏”样畸形及鼻翼沟明显变浅。满意24例,较满意8例,不满意2例,满意率为94.12%。1例因自觉鼻头肥大改善不明显经手术调整后,求美者较满意;1例因迟发性鼻小柱偏曲经手术修复后纠正;其余求美者对术后效果均满意。结论 静息角对鼻下部各亚单位美学形态具有较为重要的影响,通过静息角复位能够形成美观、稳定的鼻尖外形,值得临床推广应用。Objective To explore the clinical application and efficacy of reduction of resting angle of the lateral crura in nasal tip plasty. Methods From June 2019 to March 2021, 34 patients underwent rhinoplasty were included in this study. The autologous costal cartilage was obtained to make cartilage scaffold for rhinoplasty. The reduction of resting angle of the lateral crura was performed by lower lateral cartilage suture technique. The nasal tip shape was improved. After the operation, the efficacy was evaluated by observing whether there were problems such as "pinched nose" appearance and shallowness of alar sulcus. Results All patients were followed up for 6 to18 months after operation, with an average of 10.5 months. The height of nasal tip was obviously improved. No "pinched nose" appearance and shallowness of alar sulcus occurred. Twenty-four patients were satisfied, 8 patients were somewhat satisfied and 2 patients were dissatisfied with the results. The satisfaction rate was 94.12%. Hypertrophy of nasal tip was found in 1 patient, and the result was satisfactory after surgical adjustment. Delayed nasal columella deviation was found in 1 patient and corrected by a second operation. The remaining patients were satisfied with the postoperative efficacy. Conclusion The resting angle of the lateral crura has a great influence on the aesthetic appearance of the lower part of nose. Correcting the resting angle of the lateral crura can construct a beautiful and stable nasal tip. It should be considerable for clinical promotion and application.
