腹腔镜手术联合亮丙瑞林、戊酸雌二醇对子宫内膜异位症患者CA125及VEGF的影响

摘　　要：目的探讨腹腔镜手术联合亮丙瑞林、戊酸雌二醇在子宫内膜异位症中的应用价值。方法选取80例子宫内膜异位症患者,采用随机数字表法分为对照组与研究组,每组40例。对照组开展腹腔镜手术治疗,研究组开展腹腔镜手术联合亮丙瑞林、戊酸雌二醇治疗。比较两组患者病情症状改善情况、血管内皮生长因子(VECF)及糖类抗原125(CA125)。结果术前,两组患者视觉模拟评分法(VAS)评分、Kupperman评分比较,差异无统计学意义(P>0.05);术后3个月,研究组VAS评分低于对照组,Kuppermari评分高于对照组,差异有统计学意义(P<0.05)。术中,两组患者VEGF、CA125水平比较,差异无统计学意义(P>0.05);术后3个月,研究组VEGF、CA125水平(172.21±10.2)ng/L、(194.52±35.26)U/ml均低于对照组的(217.25±11.07)ng/L、(244.18±36.19)U/ml,差异有统计学意义(P<0.05)。结论子宫内膜异位症患者开展腹腔镜手术联合亮丙瑞林、戊酸雌二醇进行治疗,能够有效改善患者病情及腹腔液VEGF、CA125水平,值得临床推广与应用。Objective To explore the application value of laparoscopic surgery combined with leprorelin and estradiol valerate in endometriosis.Methods 80 patients with endometriosis were randomly divided into control group and study group,with 40 cases in each group.The control group was treated with laparoscopic surgery,while the study group was treated with laparoscopic surgery,leuprolide and estradiol valerate.The improvement of disease symptoms,vascular endothelial growth factor(VECF)and carbohydrate antigen 125(CA125)were compared between the two groups.Results Before operation,there was no significant difference in visual analogue scale(VAS)score and Kupperman score between the two groups(P>0.05).At 3 months after operation,the VAS score of the study group was lower than that of the control group,and the Kupperman score was higher than that of the control group,and the differences were statistically significant(P<0.05).During operation,there was no significant difference in VEGF and CA125 levels between the two groups(P>0.05).At 3 months after operation,the levels of VEGF and CA125 in the study group were(172.21±10.2)ng/L and(194.52±35.26)U/ml,which were lower than those in the control group(217.25±11.07)ng/L and(244.18±36.19)U/ml,and the difierences were statistically significant(P<0.05).Conclusion Laparoscopic surgery combined with leiorelin and estradiol valerate in the treatment of patients with endometriosis can effectively improve the condition of patients and the levels of VEGF and CA125 in peritoneal fluid,which is worthy of clinical promotion and application.
