经阴道超声诊断子宫内膜息肉的临床价值

摘　　要：目的研究针对子宫内膜息肉利用经阴道超声诊断的临床价值。方法选取115例疑为子宫内膜息肉检查患者,均行经阴道超声检查和宫腔镜病理检查,其中经病理检查确诊子宫内膜息肉患者共78例,收集相关数据资料,将病理结果作为金标准,统计诊断准确率、误诊、漏诊、诊断敏感性、特异性及影像特征等。结果经病理诊断子宫内膜息肉患者共78例,经阴道超声检查子宫内膜息肉患者71例,差异有统计学意义(P<0.05)。经阴道超声诊断准确率为91.0%、敏感性为91.0%、特异性为45.9%,阳性预测值为91.2%,阴性预测值为70.8%。病理诊断与经阴道超声检查比较,差异有统计学意义(P<0.05)。经阴道超声影像特征:71例确诊患者中,62例患者为单发息肉,9例患者为多发息肉,其息肉直径5~29mm,平均(18.6±2.5)mm。声像图显示:子宫内膜息肉主要呈水滴形、椭圆形,其分界与子宫肌层分界明显,息肉边缘清晰,表面十分光滑,部分息肉表现为带状附着于子宫腔壁上,少部分子宫息肉被内膜包绕。结论经阴道超声检查在子宫内膜息肉中的效果显著,其诊断准确率较高,可呈现清晰影像特征,直观找到病灶具体位置,为病情诊断提供科学的鉴别依据,具有临床推广价值。Objective To study the clinical value of transvaginal ultrasound in the diagnosis of endometrial polyps.Methods 115 cases of suspected patients with endometrial polyp check,vaginal ultrasound examination and hysteroscopy in pathologic examination,which confirmed by pathological examination,a total of 78 patients with endometrial polyp,collect related data,the pathological results as gold standard statistical diagnosis accuracy rate,misdiagnosis and missed diagnosis,diagnostic sensitivity,specificity and image characteristics.Results There were 78 cases of endometrial polyps diagnosed by pathology and 71 cases of endometrial polyps examined by transvaginal ultrasound,and the difference was statistically significant(P<0.05).The diagnostic accuracy,sensitivity,specificity,positive predictive value and negative predictive value of transvaginal ultrasound were 91.0%,91.0%,45.9%,91.2% and 70.8%,respectively.The difference between pathological diagnosis and transvaginal ultrasound was statistically significant(P<0.05).Among the 71 patients,62 patients had single polyps and 9 patients had multiple polyps.The diameter of polyps ranged from 5 to 29 mm,with an average of(18.6±2.5)mm.The ultrasonography showed that the endometrial polyps were mainly droplet shaped and oval,and the boundary between the polyps and the myometrium was obvious.The polyps had clear edges and a very smooth surface.Some of the polyps were banded attached to the uterine cavity wall,and a few of them were surrounded by the endometrium.Conclusion Transvaginal ultrasound has a significant effect on endometrial polyps,which has a high diagnostic accuracy,can present clear image features,intuitively find the specific location of the lesion,and provide a scientific basis for the diagnosis of the disease,and has clinical promotion value.
