摘 要:目的报告多维度交叉克氏针治疗第一掌骨基底骨折的临床疗效。方法自2016年6月至2020年5月我们采用多维度交叉克氏针内固定治疗第一掌骨基底骨折患者20例,闭合骨折12例,开放性骨折8例,骨折均未累及第一腕掌关节面,术后2周开始进行早期功能锻炼。结果伤口均Ⅰ期愈合,随访3~16个月,平均10个月。骨折愈合时间8~11周,平均9.5周。拇指各关节功能恢复良好,根据中华医学会手外科学会上肢部分功能评定试用标准评定疗效:优12例,良6例,中2例;优良率90%。结论应用多维度交叉克氏针治疗第一掌骨基底骨折,能提供初始稳定性,骨折固定相对牢固,可早期功能锻炼,术后疗效可靠。Objective To report the clinical efficacy of multi-dimensional cross Kirschner wire in the treatment of the first metacarpal base fracture.Methods From June 2016 to May 2020,20 patients with the first metacarpal base fracture were treated by multi-dimensional cross Kirschner wire internal fixation.There were 12 closed fractures and 8 open fractures.The fractures did not involve the first carpometacarpal articular surface,and early functional exercise began 2 weeks after operation.Results All the wounds achieved primary healing.The follow-up time ranged from 3 to 16 months,with an average of 10 months.The fracture healing time was 8 to 11 weeks,with an average of 9.5 weeks.The function of each joint of thumb recovered well.According to the trial standard of upper limb function evaluation issued by Hand Surgery Society of Chinese Medical Association,the results were evaluated as excellent in 12 cases,good in 6 cases and fair in 2 cases.The excellent and good rate was 90%.Conclusion The application of multi-dimensional cross Kirschner wire in the treatment of the first metacarpal base fracture can provide initial stability,relatively firm fracture fixation,early functional exercise and reliable postoperative effect.
