摘　　要：目的探讨应用肌腱转位分期重建前臂完全离断再植术后手内肌功能的临床效果。方法自2012年2月至2018年6月,我们对5例前臂完全离断再植术后手内肌功能障碍患者,分期采用转位示小指指浅屈肌腱重建蚓状肌功能,转位尺侧腕伸肌腱联合掌长肌腱重建拇对掌功能。术后早期开始功能锻炼,并进行定期随访。结果术后5例患者伤口均Ⅰ期愈合,随访时间为6~26个月,平均13个月。手指屈伸及拇指对掌功能恢复良好,根据中华医学会手外科学会上肢部分功能评定试用标准评定手功能:优3例,良1例,可1例。结论采用肌腱转位分期重建前臂完全离断再植术后手内肌功能可以获得较满意的疗效。Objective To investigate the clinical efficacy of tendon transposition in the staged reconstruction of hand intrinsic muscle function after completely severed forearm replantation.Methods Form February 2012 to June 2018,5 patients with hand intrinsic muscle dysfunction after completely severed forearm replantation was treated.The lumbrical muscle function was reconstructed by transposition of the superficial flexor tendon of the index and little finger,and the thumb opposition function was reconstructed by transposition of the extensor carpi ulnaris tendon combined with the palmaris longus tendon.The functional exercise was started early after operation and the patients were followed up regularly.Results The wounds of 5 patients achieved primary healing.The follow-up time ranged from 6 to 26 months,with an average of 13 months.The finger flexion and extension and thumb opposition function recovered well.According to the upper limb function evaluation trial standard issued by Hand Surgery Society of Chinese Medical Association,the results were rated as excellent in 3 cases,good in 1 case and fair in 1 case.Conclusion The application of tendon transposition in the staged reconstruction of hand intrinsic muscle function after completely severed forearm replantation can obtain satisfactory results.
