PFNA内固定联合康复指导治疗股骨粗隆间骨折的临床研究

摘　　要：目的研究股骨近端防旋髓内钉(PFNA)内固定联合康复指导治疗股骨粗隆间骨折的临床效果。方法选取80例于2019年5月-2020年12月间在安远县人民医院接受治疗的股骨粗隆间骨折患者为研究对象,以随机数字表法将其分为2组,即:对照组(n=40)与康复组(n=40)。2组均进行PFNA内固定治疗,对照组经医护人员口头说明术后锻炼的原则及方式后由患者自主进行功能锻炼,康复组则给予系统化的康复指导。比较2组患者治疗后的髋关节功能、日常生活能力、骨折恢复情况及并发症发生率。结果2组治疗前Harris评分、BI评分相比,差异无统计学意义(P>0.05);治疗后1、3、6个月康复组Harris评分、BI评分均高于对照组,差异具统计学意义(P<0.05)。相较于对照组,康复组负重时间及骨折愈合时间更短,并发症发生率更低,差异具统计学意义(P<0.05)。结论股骨粗隆间骨折采用PFNA内固定治疗的同时给予康复指导,可加快骨折康复,有利于提升髋关节功能及自理能力,同时有效降低并发症,值得临床借鉴。Objective To study the clinical effect of proximal femoral anti rotation intramedullary nail(PFNA)internal fixation combined with rehabilitation guidance in the treatment of femoral intertrochanteric fracture.Methods 80 patients with femoral intertrochanteric fracture treated in Anyuan County People’s Hospital from May 2019 to December 2020 were selected as the research object. They were randomly divided into two groups:control group(n=40)and rehabilitation group(n=40). Both groups were treated with PFNA internal fixation. The control group performed functional exercise independently after the medical staff orally explained the principles and methods of postoperative exercise,while the rehabilitation group was given systematic rehabilitation guidance.The hip function,activities of daily living,fracture recovery and complication rate were compared between the two groups.Results There was no significant difference in Harris score and Bi score between the two groups before treatment(P>0.05);The Harris score and Bi score of the rehabilitation group were higher than those of the control group at 1,3 and 6 months after treatment(P<0.05). Compared with the control group,the weight-bearing time and fracture healing time of the rehabilitation group were shorter,and the incidence of complications was lower,the difference was statistically significant(P<0.05).Conclusion The treatment of femoral intertrochanteric fracture with PFNA internal fixation and rehabilitation guidance can accelerate the rehabilitation of fracture,promote the improvement of hip function and self-care ability,and effectively reduce complications,which is worthy of clinical reference.
