彩色多普勒超声心动图诊断原发性心脏肿瘤的临床应用价值

摘　　要：目的:研究分析彩色多普勒超声心动图(CDE)诊断原发性心脏肿瘤的临床应用价值。方法:研究自医院抽取20例入院治疗心脏肿瘤患者,时间为2020年4月至2021年4月,观察CDE的影像学特点及诊断价值。结果:CDE诊断结果显示:总计18例,4例为原发性恶性肿瘤、3例非黏液良性瘤、11例黏液瘤。以病理诊断结果为金标准下,对20例患者行B超诊断,阳性患者为4例,其中4例与病理诊断结果相同,符合率95.00%,其敏感度为80.00%,特异度为93.33%,阳性率为80.00%,阴性率为93.33%。CDE检出18例原发性心脏肿瘤患者里,8例为左房黏液瘤。3例为室间隔纤维瘤。结论:常规影像学能够诊断出心脏内占位病变,大多数可观察到心脏肿瘤,但分型不够明确;CDE能够确定患者的心功能变化,CDE能够较好地显示肿瘤位置、数目、直径等特征,能够为临床诊断疾病提供可靠的影像学参考,有利于手术方案实施。肿瘤病灶较大及血流动力学影响较高的肿瘤可能导致患者心功能下降,因此还需使用CDE来进行确诊,并使用手术切除或者部分切除手术帮助患者恢复心功能。Objective:To study and analyze the clinical application value of color Doppler echocardiography(CDE)in the diagnosis of primary cardiac tumors.Methods:20 patients with cardiac tumors were selected from the hospital from April 2020 to April 2021,and the imaging features and diagnostic value of CDE were observed.Results:CDE diagnosis showed that there were 18 cases in total,4 cases were primary malignant tumors,3 cases were non-mucinous benign tumors and 11 cases were myxoma.With pathological diagnosis as the gold standard,20 patients were diagnosed by B-ultrasound,of which 4 were positive,4 were the same as pathological diagnosis,the coincidence rate was 95.00%,the sensitivity was 80.00%,the specificity was 93.33%,the positive rate was 80.00%and the negative rate was 93.33%.Of the 18 patients with primary cardiac tumors detected by CDE,8 were left atrial myxoma.Cases were ventricular septal fibroma.Conclusion:Conventional imaging can diagnose space occupying lesions in the heart,and most of them can observe cardiac tumors,but the classification is not clear enough.CDE can determine the changes of patients'cardiac function,and CDE can better display the features of tumor location,number,diameter,etc.It can provide reliable imaging reference for clinical diagnosis of diseases,which is beneficial to the implementation of surgical plan.Tumors with large lesions and high hemodynamic effects may lead to the decline of patients'cardiac function,so CDE is needed to make a diagnosis,and surgical resection or partial resection is used to help patients recover their cardiac function.
