摘  要:目的:评价CT多平面重组技术(MPR)优选的穿刺路径对CT引导下经皮肺穿刺肺结节效果的影响。方法:回顾性分析2020年1月至2022年4月收治的52例CT引导下经皮肺穿刺手术患者的资料。其中普通经皮肺穿刺组24例(A组),MPR经皮肺穿刺组28例(B组)。对比两组病灶穿刺调整次数、穿刺针与水平面夹角、穿刺用时、并发症发生率及一次穿刺成功率。结果:两组穿刺调整次数、穿刺时间比较,差异有统计学意义(P<0.05),B组平均减少0.4次,缩短5 min。B组一次穿刺成功率及术后并发症发生率与A组相当(P>0.05),两组均未出现极严重并发症。B组穿刺路径与水平面夹角较A组更小(P<0.05)。结论:多层螺旋CT多平面重组技术优选的穿刺路径可以减少穿刺过程的调整次数,缩短操作时间,对肺结节CT引导穿刺路径的选择有一定指导价值。Objective:To evaluate the clinical application value of multi-planar reformation(MPR)technique in CT-guided percutaneous lung puncture path selection for pulmonary nodules.Methods:The clinical data of 52 patients who underwent CT-guided percutaneous lung puncture in our hospital between January 2020 and April 2022 were retrospectively reviewed.There were 24 cases in the general percutaneous lung puncture group(group A)and 28 cases in the MPR percutaneous lung puncture group(group B).The differences in the number of lesion punctures adjustments,the angle formed by the puncture needle and the horizontal plane,the puncture time,the complication rate,the primary puncture success rate between the two groups were compared.Results:The differences in the number of puncture adjustments and puncture time between the two groups were statistically significant(P<0.05),and the average reduction in group B was 0.4 times,and shortened 5 minutes.The primary puncture success rate and the postoperative complication rates in group B were comparable to those in group A(P>0.05),there were no serious complications in either group.The angle between puncture path and horizontal plane in group B was smaller than that in group A(P<0.05).Conclusion:The optimal puncture path of multi-slice spiral CT with multi-plane reconstruction technique can reduce the adjustment times of the puncture process,shorten the operation time,and has a guiding value for the selection of CT guided puncture path of pulmonary nodules.
