左心衰患者血清MR-ProANP、sST2水平与心室重构、心功能关系研究

摘　　要：目的:探讨与研究左心衰患者血清心房钠尿肽前体中肽段(MR-proANP)、可溶性生长刺激表达基因2蛋白(sST2)水平与心室重构、心功能的关系。方法:选择2020年5月至2022年1月于医院进行诊治的左心衰患者80例为左心衰组,选择同期健康人群80例为健康组。检测两组MR-ProANP、sST2水平并进行心室重构检查,判定冠心病患者的心功能并进行相关性分析。结果:左心衰组的血清MR-ProANP、sST2含量明显高于健康组,P<0.05。左心衰组的左室舒张末期容积(LVEDV)、左室收缩末期容积(LVESV)明显高于健康组,P<0.05;左室射血分数(LVEF)明显低于健康组,P<0.05。在左心衰组中,纽约心脏病协会(NYHA)心功能分级为Ⅰ级32例、Ⅱ级28例、Ⅲ级13例、Ⅳ级7例。Pearson法分析显示,在左心衰组中,血清MR-ProANP、sST2含量与LVEDV、LVESV、LVEF、心功能分级存在相关性,P<0.05。多因素Logistic回归分析显示血清MR-ProANP、sST2含量为影响患者心功能分级的重要因素,P<0.05。结论:左心衰患者血清MR-ProANP、sST2呈现高表达状况,血清MR-ProANP、sST2水平与心室重构、心功能都存在相关性。Objective:To investigate the relationship between the levels of serum atrial natriuretic peptide precursor middle peptide(MR-ProANP),soluble growth stimulation expressed gene 2 protein(sST2)and ventricular remodeling and cardiac function in patients with left heart failure.Methods:A total of 80 patients with left heart failure who were diagnosed and treated in the hospital from May 2020 to January 2022 were selected as left heart failure group,and 80 healthy people were selected as healthy group during the same period.The levels of MR-ProANP and sST2 in the two groups were detected,and the ventricular remodeling was examined to determine the cardiac function of patients with coronary heart disease and analyze the correlation.Results:The levels of serum MR-ProANP and sST2 in the left heart failure group were significantly higher than those in healthy group(P<0.05).The left ventricular end diastolic volume(LVEDV)and left ventricular end systolic volume(LVESV)in the left heart failure group were significantly higher than those in the healthy group(P<0.05);The left ventricular ejection fraction(LVEF)was significantly lower than that of the healthy group(P<0.05).In the left heart failure group,the New York Heart Association(NYHA)cardiac function classification was gradeⅠwith 32 cases,gradeⅡwith 28 cases,gradeⅢwith 13 cases and gradeⅣwith 7 cases.In the left heart failure group,Pearson's analysis showed that serum MR-ProANP and sST2 levels were correlated with LVEDV,LVESV,LVEF and cardiac function grade(P<0.05).Multivariate Logistic regression analysis showed that serum levels of MR-ProANP and sST2 were important factors affecting the cardiac function classification of patients(P<0.05).Conclusion:Serum MR-ProANP and sST2 are highly expressed in patients with left heart failure,and serum MR-ProANP and sST2 levels are correlated with ventricular remodeling and cardiac function.
