长时程动态心电图在心律失常事件筛查中的应用价值

摘　　要：目的比较长时程动态心电图与标准12导联心电图心律失常事件检出率,探讨长时程动态心电图在心律失常事件筛查中的应用价值。方法选取2018年7月~2021年5月内蒙古某煤电公司职员2267人,经标准12导联心电图排查,筛选1964人进行长时程动态心电图检查,比较长时程动态心电图(24 h、7 d)与标准12导联心电图对心律失常事件的检出率。结果与标准12导联心电图比较,长时程动态心电图在室性心动过速、室上性心动过速、阵发性心房颤动及高度房室传导阻滞等心律失常事件中检出率明显增高,差异有统计学意义(P<0.001);心律失常事件检出率随记录时间的延长具有逐渐增高的趋势。结论长时程动态心电图可明显提高阵发性心律失常事件的检出率,心律失常事件检出率随记录时间的延长有逐渐增高的趋势。Objective To compare the detection rate of arrhythmia events between long term ambulatory electrocardiogram(ECG)and standard 12-lead ECG,and to explore the application value of long term ambulatory ECG in arrhythmia events screening.Methods A total of 2267 employees of a coal power company in Inner Mongolia from July 2018 to May 2021 were included,and 1964 of them were screened by standard 12-lead ECG,and the detection rate of arrhythmia events in long term ambulatory ECG(24 h,7 d)and standard 12-lead ECG were compared.Results Compared with standard 12-lead ECG,the detection rate of ventricular tachycardia,supraventricular tachycardia,paroxysmal atrial fibrillation,high atrioventricular block and other arrhythmia events of long term ambulatory ECG was significantly increased(P<0.001).The detection rate of arrhythmia events increased gradually with the prolongation of recording time.Conclusion The detection rate of paroxysmal arrhythmia can be significantly increased by long term ambulatory ECG and the detection rate of paroxysmal arrhythmia gradually increases with the prolongation of recording time.
