应对方式对类风湿性关节炎患者抑郁情绪的影响:希望水平的中介效应模型

摘　　要：目的探讨应对方式对类风湿性关节炎(rheumatoid arthritis,RA)缓解期患者抑郁情绪的影响。方法采用Herth希望量表(Herth Hope Scale)、医学应对问卷(Medical Coping Questionnaire)、流调用抑郁问卷(catchment-area epidemiology survery-depression,CES-D)量表,对2019年9月~2021年2月处于类风湿性关节炎疾病缓解期的187例患者进行问卷调查。结果本调查中187例RA患者的抑郁情绪均值为(25.80±15.09)分,疾病缓解期RA患者中有抑郁情绪者占69%,发生率偏高。RA患者的应对方式(屈服、回避)可正向预测抑郁情绪(r=0.73,P<0.01;r=0.661,P<0.01)。应对方式(屈服)对抑郁情绪的影响中,希望水平中介效应显著,95%CI[3.3248,4.3707]。应对方式(屈服)对抑郁情绪的效应在引入中介变量后,直接效应显著,95%CI[0.6224,1.2287]。在应对方式(回避)对抑郁情绪的影响中,希望水平中介效应显著,95%CI[2.7595,3.8487]。应对方式(回避)对抑郁情绪的影响在引入中介变量后,直接效应显著,95%CI[0.7223,1.5683]。结论应对方式(屈服、回避)除直接影响RA患者的抑郁情绪外,还通过希望水平的中介作用间接影响RA患者的抑郁情绪状况。Objective To explore the influence of coping style on depression in rheumatoid arthritis(RA)patients in remission.Methods The Herth Hope Scale,Medical Coping Questionnaire and catchment-area epidemiology survery-depression(CES-D)scale were used to investigate 187 patients with rheumatoid arthritis in remission from September 2019 to February 2021.Results It was found that the mean depressive mood score of the 187 RA patients was(25.80±15.09),and 69%of RA patients in remission stage had a high incidence of depressive mood.The coping style(yielding,avoidance)of RA patients positively predicted depression(r=0.73,P<0.01;r=0.661,P<0.01).Among the effects of coping style(yielding)on depression,the level of hope had a significant mediating effect,95%CI[3.3248,4.3707].The effect of coping style(yielding)on depressive mood was significant after the introduction of mediator variables,with 95%CI[0.6224,1.2287].In the influence of coping style(avoidance)on depression,the level of hope had a significant mediating effect,95%CI[2.7595,3.8487].The effect of coping style(avoidance)on depressive mood was significant after the introduction of mediator variables,with a 95%CI[0.7223,1.5683].Conclusion Coping style(yielding,avoidance)not only directly affects the depressive mood of RA patients,but also indirectly affects the depressive mood of RA patients through the mediating effect of hope level.
