摘　　要：目的探讨腹部多个宽蒂超薄皮瓣修复多手指创面的临床效果。方法自2016年1月至2020年12月我们治疗多手指创面共20例51指, 均为单侧手多指掌侧或背侧创面;指掌侧创面8例, 指背创面12例;其中11例3指创面, 9例2指创面;单一创面面积1.0 cm×2.0 cm~3.0 cm×7.0 cm。均采用同侧腹部多个宽蒂超薄皮瓣修复多指创面, 14~18 d断蒂。结果术后1例其中一块皮瓣因缝合张力大, 远端部分坏死, 经换药后痊愈;其余病例皮瓣均完全存活。术后随访3~20个月, 皮瓣外观无臃肿, 质地良好。参照中华医学会手外科学会手部肌腱修复后评定标准:优35指, 良9指, 可5指, 差2指。结论应用腹部多个宽蒂超薄皮瓣可同时修复多手指创面, 皮瓣成活率高, 外形质地良好, 手功能恢复满意。Objective To investigate the clinical efficacy of abdominal multiple wide pedicled ultra-thin flaps for repair of multiple finger wounds.Methods From January 2016 to December 2020,20 patients(51 fingers)with multi finger wounds were treated,all of which were volar or dorsal wounds of unilateral multi finger hands.There were 8 cases of volar digital wounds and 12 cases of dorsal digital wounds.Among them,11 cases had wounds in 3 fingers and 9 cases had wounds in 2 fingers.The single wound area ranged from 1.0 cm×2.0 cm to3.0 cm×7.0 cm.The ipsilateral abdominal multiple wide pedicled ultra-thin flaps were used to repair the wound of multiple fingers,and the pedicle was broken in 14 to 18 days.Results After operation,one of the flaps was cured after dressing change because of large suture tension and partial necrosis of the distal end.The flaps of other cases survived completely.The flap was follow-up for 3 to 20 months.The appearance of the flap was not bulky and the texture was good.According to the evaluation standard of hand.tendon repair issued by Hand Surgery Society of Chinese Medical Association,the results were rated as excellent in 35 fingers,good in 9 fingers,fair in 5 fingers and poor in 2 fingers.Conclusion The application of abdominal multiple wide pedicled ultra-thin flaps can repair the wound of multiple fingers at the same time.The survival rate of the flap is high,the appearance and texture are good,and the recovery of hand function is satisfactory.
