卢秉久教授辨治原发性胆汁性胆管炎用药经验与规律研究

摘　　要：目的:采用数据挖掘方法分析卢秉久教授针对原发性胆汁性胆管炎(PBC)的辨治经验及用药规律。方法:通过临床病例采集建立医案数据库,运用频数统计、关联规则分析、聚类分析、复杂系统熵聚类、熵层析聚类分析等高级别数据挖掘方法,探讨卢秉久教授治疗PBC的用药特点和组方规律。结果:纳入100例病例,共筛选出有效方剂100条,使用132种药物,累计频次为1 169次。使用频次高于30的中药有22味。药性频次从高到低依次为温性、凉性、平性、寒性、热性;药味依次为甘味、辛味、咸味、酸味、苦味、涩味;归经主要入脾、肝、胆经,其次为胃、肺、肾、膀胱经。使用频次在30次以上的核心药组共15组,18组可以用于新处方聚类的药物组合及新处方9首。结论:卢秉久教授治疗PBC主要以祛湿为主,围绕祛湿,可采取健脾渗湿、活血祛湿、温阳化湿、解表散湿、行气祛湿、清热燥湿、淡渗利湿等方法。为进一步总结卢秉久教授对于PBC治疗经验及探寻新药处方提供参考依据。Objective:Based on the "v2.5" of traditional Chinese Medicine Inheritance assistant platform management system, we can find out professor Lu Bingjiu’s experience in differentiation and treatment of primary bile cholangitis(hereinafter referred to as PBC) and the rule of medication.Methods:The medical record database was established by clinical case collection, and high-level data mining methods such as frequency statistics, association rule analysis, cluster analysis, complex system entropy cluster analysis, and entropy chromatography cluster analysis were used to explore the medication characteristics and prescription rules of Professor Lu Bingjiu in the treatment of PBC. Results:100 cases were included, while 100 effective prescriptions were selected, 132 drugs were used and the cumulative frequency was 1 169.There were 22 herbs used more than 30 times. The frequency of drug resistance was temperature, cool, flat, cold and hot in order from high to low;the taste of medicine was sweet, spicy, salty, sour, bitter and astringent;the main components of the medicine are spleen, liver and bile. The core drug group with frequency of more than 30 times was 15 groups, 18 groups could be used for clustering new prescriptions and 9 new prescriptions. Conclusion:Professor Lu Bingjiu can treat PBC mainly by removing dampness, and can take the methods of invigorating spleen, promoting blood circulation, warming yang and removing moisture, removing surface and dispersing moisture, removing Qi and removing dampness, clearing heat, drying and dampness, and removing moisture by light infiltration and diuresis. This study provides a reference for the study of the experience of Professor Lu Bingjiu in the treatment of PBC and the exploration of new drug prescription.
