摘  要:目的 分析超声剪切波弹性成像(SWE)联合乳腺超声诊断标准(BI-RADS-US)对乳腺小肿块各向异性及良恶性的评估价值。方法 采用回顾性分析法,选取接受常规超声及SWE检查的120例乳腺小肿块患者(研究组)和30例健康女性(对照组)作为研究对象,获取弹性模量参数并计算Asp值,记录BI-RADS-US分类。分析Asp、BI-RADS-US单用及联用对恶性乳腺肿块的诊断效能。结果 E_(max)、E_(min)、E_(mean)、SD、Asp值均有恶性组>良性组>对照组,组间比较差异有统计学意义(P<0.05)。Asp诊断乳腺癌的AUC值为0.823,明显高于其他参数(P<0.05)。与BI-RADS-US分类诊断结果相比,BI-RADS-US分类联合Asp对乳腺恶性肿块的诊断特异度、准确度及阳性预测值更高(P<0.05)。结论 SWE模量可定量分析乳腺肿瘤组织的各向异性,各向异性弹性模量参数联合BI-RADS-US分类可有效提高乳腺小肿块良恶性的诊断特异度及准确度。Objective To analyze the value of ultrasonic shear wave elastography(SWE)combined with breast ultrasound diagnostic criteria (BI-RADS-US)in the evaluation of anisotropy,benign and malignant of small breast masses.Methods 120patients with small breast masses(Study Group)and 30healthy women(control group)who underwent routine ultrasound and SWE were selected as the research objects by retrospective analysis.The elastic modulus parameters were obtained,the ASP value was calculated,and the BI-RADS-US classification was recorded.The diagnostic efficacy of ASP and BI-RADS-US alone and in combination in malignant breast masses were analyzed.Results The values of E_(max),E_(min),E_(mean),SD and ASP in malignant group> benign group > control group.The difference between the groups was statistically significant(P<0.05).The AUC value of Asp in diagnosing breast cancer was 0.823,which was significantly higher than that of other parameters(P<0.05).Compared with BI-RADS-US classification,BI-RADS-US classification combined with ASP has higher specificity,accuracy and positive predictive value in the diagnosis of breast malignant masses(P<0.05).Conclusions SWE modulus can quantitatively analyze the anisotropy of breast tumor tissue.Anisotropic elastic modulus parameters combined with BI-RADS-US classification can effectively improve the specificity and accuracy of diagnosis of benign and malignant small breast masses.
