摘 要:目的 探讨彩超辅助下儿童尺桡骨骨折手法复位并石膏外固定术的可行性。方法 选取确诊尺桡骨骨折患儿共60例。将患儿分为超声组、对照组,每组30例。超声组使用彩超辅助下闭合复位石膏固定,对照组采用常规手法复位石膏固定,比较两组闭合复位成功率、初次闭合复位成功率、骨折愈合时间、并发症发生情况以及康复后前臂功能恢复情况。结果 超声组初次闭合复位成功率明显高于对照组,愈合时间明显短于对照组,差异有统计学意义(P<0.05)。两组闭合复位成功率、延迟愈合发生率、畸形愈合发生率、再骨折发生率、术后前臂Anderson功能评分等,差异无统计学意义(P>0.05)。结论 儿童前臂骨折手法复位石膏固定中应用彩超辅助,可提高初次闭合复位成功率,缩短骨折愈合时间,且无放射性损伤,值得临床推广应用。Objective:To investigate the feasibility of manual reduction and plaster external fixation of ulna and radius fractures in children assisted by color doppler ultrasound and manual reduction and intramedullary pin fixation.Methods:Sixty cases of ulna and radius fracture were diagnosed by x-ray.The children were divided into A and B groups.Closed reduction and plaster fixation(Group A,n = 30)were performed with the aid of color doppler ultrasound,and compared the success rate of closed reduction,the success rate of primary closed reduction,the healing time of fracture,the complications and the recovery of forearm function with Group B of conventional manual reduction and plaster fixation(Group B,n = 30).Results:In the 60 cases of forearm fracture treated by manual reduction and plaster fixation,there were significant differences in the success rate of primary closed reduction,the healing time between Group A and the control group(P<0.05).There was no significant difference in the success rate of closed reduction,the rate of delayed healing,the rate of malunion,the rate of refracture and the Anderson Function Score(P>0.05).Conclusion:The application of color doppler ultrasound in the manipulative reduction and plaster fixation of forearm fracture in children can improve the success rate of the first closed reduction and shorten the time of fracture healing.The advantage of no radiation injury is worthy of clinical study.
