照护者自我护理贡献水平对慢性心力衰竭病人自我护理行为影响的研究

摘　　要：目的:了解慢性心力衰竭(chronic heart failure,CHF)病人自我护理行为和心力衰竭照护者自我护理贡献水平,探讨心力衰竭照护者自我护理贡献水平对CHF病人自我护理行为的独立影响。方法:方便抽取127例CHF病人及其照护者为研究对象,采用心力衰竭病人自我护理行为量表、心力衰竭自我护理贡献量表进行调查。结果:CHF病人的自我护理行为总分为(61.60±11.31)分,其中自我护理维持(24.05±4.85)分,自我护理管理(20.94±4.64)分,自我护理信心(16.57±5.30)分;心力衰竭照护者自我护理贡献总分为(60.46±10.29)分,其中自我护理维持(23.94±4.63)分,自我护理管理(20.47±5.10)分,自我护理信心(16.06±4.70)分。多元线性分层回归分析显示,控制CHF病人年龄、性别、文化程度、职业、婚姻状况、人均月收入、居住地、居住方式、美国纽约心脏病学会(NYHA)心功能分级、因心力衰竭住院总次数等混杂因素后,心力衰竭照护者自我护理贡献水平是CHF病人自我护理行为的独立影响因素(β=0.351,ΔR^(2)=0.072,P<0.001)。结论:心力衰竭照护者自我护理贡献水平独立影响CHF病人的自我护理行为,因此临床医务人员应评估心力衰竭照护者的自我护理贡献水平,针对性地开展个体化的干预措施,以提高CHF病人的自我护理行为。Objective:To investigate the level of self-care behavior in patients with chronic heart failure(CHF)and the level of self-care contribution of heart failure caregivers,and to explore the independent effect of self-care contribution level of the caregivers on self-care behavior of CHF patients.Methods:A total of 127 patients with CHF and their caregivers were conveniently selected for the study and investigated using the Self-Care Behavior Scale of Heart Failure Patient and the Self-Care Contribution Scale of Heart Failure.Results:The total score of self-care behavior of CHF patients was(61.60±11.31).The scores of self-care maintenance,self-care management,and self-care confidence were(24.05±4.85),(20.94±4.64)and(16.57±5.30),respectively.The total score of self-care contribution of heart failure caregivers was(60.46±10.29).The scores of self-care maintenance,self-care management,and self-care confidence were(23.94±4.63),(20.47±5.10)and(16.06±4.70),respectively.Multivariate linear hierarchical regression analyses showed that the level of self-care contribution of heart failure caregivers independently predicted the self-care behavior among patients with CHF after controlling for confounding factors such as age,gender,education level,occupation,marital status,per capita monthly income,place of residence,living style,New York Heart Association(NYHA)cardiac function class,and total number of hospitalizations due to heart failure(β=0.351,ΔR^(2)=0.072,P<0.001).Conclusions:The level of self-care contribution of the caregivers independently affects the self-care behavior in patients with CHF.Therefore,clinicians should evaluate the level of self-care contribution of the caregivers and carry out individualized interventions for improving the self-care behavior of patients with CHF.
