摘 要:目的:比较多维锁定(Multiloc)髓内钉和锁定解剖(Philos)钢板治疗肱骨近端骨折的临床疗效。方法:回顾我科自2018年1月-2021年9月期间收治的126例Neer2、3部分肱骨近端骨折病例,其中56例采用髓内钉固定治疗(髓内钉组),70例采用锁定钢板固定治疗(钢板组)。比较2组手术切口长度、术中出血量、手术时间、骨折临床愈合时间、术后12月肩关节VAS疼痛评分和Constant-Murley功能评分。结果:126例患者均获得6~12个月随访,髓内钉组在切口长度、出血量、手术时间方面优于钢板组,差异具有统计学意义(P<0.05);2组在骨折愈合时间、肩关节疼痛和功能评分中的差异无统计学意义(P>0.05)。结论:多维锁定髓内钉和解剖锁定钢板在部分不稳定型肱骨近端骨折的治疗中均可取得满意疗效,但相较而言,髓内钉组切口更小、出血更少、手术时间更短,更符合微创的理念。Objective To compare the clinical efficacy of multi dimension locking intramedullary nail(Multiloc) and locking anatomical plate(Philos) in the treatment of proximal humeral fractures. Methods We reviewed 126 cases of NEER2 and 3 proximal humerus fractures treated in our department from January 2018 to September 2021. Among them, 56 cases were treated with intramedullary nailing(Multiloc group) and 70 cases with locking plate(Philos group). The length of incision, intraoperative bleeding, operative time, fracture healing time, shoulder joint VAS pain score and Constant-Murley function score at 12 months after operation were compared between the two groups. Results All 126 patients were followed up for 6-12 months. The intramedullary nail group was superior to the steel plate group in incision length, bleeding volume and operation time, with a statistically significant difference(P<0.05). There was no significant difference in healing time, shoulder pain and functional score between the two groups(P>0.05). Conclusion Multi-dimensional locking intramedullary nail and anatomical locking plate can achieve satisfactory results in the treatment of partial unstable proximal humeral fractures, but in comparison, intramedullary nail group has smaller incision, less bleeding, and shorter operation time, which is more consistent with the concept of minimally invasive.
