摘　　要：目的:探讨外周血淋巴细胞百分率与乳酸脱氢酶(lactic dehydrogenase,LDH)、羟丁酸脱氢酶(hydroxybutyrate dehydrogenase,HBDH)联合检测在儿童传染性单核细胞增多症(infectious mononucleosis,IM)的早期筛检预估价值。方法:收集2014-2021年十堰市人民医院确诊IM患儿325例,将其设为病例组,同期随机抽取确诊为支气管肺炎或急性上呼吸道感染330例患儿设为对照组。分析淋巴细胞百分率与LDH、HBDH单独及联合筛查的效能。结果:联合筛查时的曲线下面积(AUC)及约登指数优于三项单独筛查,具有更好的筛查效能。结论:在早期IM筛查中,外周血淋巴细胞百分率、LDH和HBDH的联合检测是一种简单有效的评估方法,有助于基层医生及时辨别IM,可用于临床诊疗。Objective To explore the predictive value of combined detection of peripheral blood lymphocyte percentage,lactate dehydrogenase(LDH)and hydroxybutyrate dehydrogenase(HBDH)in the early screening of children with infectious mononucleosis(IM).Methods 325 children diagnosed with infectious mononucleosis in Shiyan People’s Hospital from 2014 to 2021 were selected as the case group,and 330 children diagnosed with bronchopneumonia or acute upper respiratory tract infection during the same period were randomly selected as the control group.The percentage of lymphocytes,the efficacy of LDH and HBDH screening alone and jointly were analyzed.Results The area under curve(AUC)and Youden index of combined screening were better than those of three separate screening,and had better screening performance.Conclusion In the early screening of IM,the combined detection of peripheral blood lymphocyte percentage,LDH and HBDH is a simple and effective assessment method,which is helpful for primary doctors to identify IM in time,and can be used for clinical diagnosis and treatment.
