摘 要:目的比较多层螺旋CT(MSCT)与锥形束CT(CBCT)在测量耳蜗及面神经管径线中的差别,并探讨其临床应用价值.方法选择临床检查(包括耳镜、纯音测听)及MSCT或CBCT检查耳部未见异常的60例受试者(30例MSCT及30例CBCT),分别测量耳蜗长度、高度,耳蜗底转长度、高度,中顶转及底转中点的垂直距离,蜗神经管直径,面神经管迷路段的长度、宽度,迷路段与耳蜗、前庭的距离,迷路段与耳蜗长轴的夹角,面神经管第1膝角度.分别比较上述结构左、右侧MSCT/CBCT的测量结果,并比较上述结构在MSCT与CBCT间测量结果的差异.结果MSCT、CBCT均可清晰显示耳蜗、面神经管等,上述结构左、右侧MSCT/CBCT的测量结果及上述结构MSCT与CBCT间测量结果比较,差异无统计学意义(P>0.05).结论与MSCT相比,应用CBCT测量耳蜗及面神经管的大小与MSCT测量结果一致,该测量结果均可作为标准以判断相应结构是否发育畸形,为临床诊疗提供客观依据.Objective To compare the sizes of cochlea and facial nerve canal in multi-slice CT(MSCT)and cone-beam CT(CBCT),and to explore the clinical application values of these normal structures.Methods The MSCT or CBCT scan was per-formed in 60 patients(30 under MSCT-scan,30 under CBCT-scan)who showed no obvious ear abnormality in clinical examination,including ear endoscopy and pure tone audiometry.The length and height of the cochlea,the length and height of the basal turn of cochlea,the distance between the midpoint of middle-apical turns and basal turn,the diameter of the cochlear canal,the length and width of the labyrinthine segment of the facial nerve canal,the distance between the labyrinthine segment of the facial nerve canal.and cochlea,vestibule,the angle between the la byrinthine segment of the facial nerve canal and long axis of cochlea and the angle of.facial nerve canal first genu were measured,respectively.The measurements of these structures between the two sides in CBCT or MSCT were compared respectively,and that between CBCT and MSCT were compared.Results The cochlea and facial nerve canal could be clearly visualized by MSCT and CBCT.There were no significant statistical differences for the measurements of these struc-tures between the two sides in CBCT or MSCT,and that between CBCT and MSCT(P>>0.05).Conclusion Compared with the MSCT,the measurements of the sizes of the cochlea or facial nerve canal in CBCT are consistent.Therefore,these measurements can be a standard to judge whether the corresponding structure is malformed,and can provide the reliable evidence for clinical treatment.
