椎间孔镜BEIS技术治疗老年性腰椎间盘突出症的临床体会

摘　　要：目的:探讨椎间孔镜BEIS(Broad Easy Immediate Surgery)技术治疗老年性腰椎间盘突出症的临床效果。方法:纳入2017年1月~2020年3月本院骨科收治的≥60岁的腰椎间盘突出症患者48例为观察对象,其中,L3-4患者2例,L4-5患者35例,L5-S1患者11例,男性30例,女性18例,以椎间孔镜BEIS技术进行治疗,采用视觉模拟评分法(VAS法)和改良MacNab疗效评定标准评定治疗效果(评估每例患者术前和术后即刻、72h、1周、3个月腿痛VAS、腿痛VAS评分改善值,评估术后1年基于患者手术经历和术后疗效的问卷调查及改良MacNab标准的优良率)。结果:术后即刻、72h、1周、3个月腿痛VAS评分均显著低于术前、腿痛VAS评分改善值显著,差距具有统计学意义(P<0.05),评估改良MacNab标准的优良率,本组优39,良5例,可4例优良率91.7%(44/48)。结论:经皮椎间孔镜BEIS技术治疗老年性腰椎间盘突出症近期效果优良,创伤小,恢复快。Objective:To investigate the clinical effect of foraminal endoscopic BEIS technology in the treatment of senile lumbar disc herniation.Methods:48 patients with lumbar disc herniation≥60 years old who were admitted to the Department of Orthopaedics of our hospital from January 2017 to March 2020 were included as the observation objects,including 2 patients with L3/4,35patients with L4/5,11 patients with L5/S1,30 males and 18 females,were treated with foraminal lens BEIS technology,Visual analogue scale(VAS)and modified macnab efficacy evaluation standard were used to evaluate the treatment effect(evaluate the vas and VAS scores of leg pain of each patient immediately,72 hours,1 week and 3 months before and after operation,and evaluate the questionnaire survey based on the patient’s operation experience and postoperative efficacy and the excellent and good rate of modified macnab standard one year after operation).Results:The VAS score of leg pain immediately,72 hours,1 week and 3 months after operation was significantly lower than that before operation.The improvement value of VAS score of leg pain was significantly different(P<0.05).The excellent and good rate of the improved macnab standard was evaluated.In this group,39 cases were excellent,5 cases were good and 4 cases were fair.The excellent and good rate was 91.7%(44/48).Conclusion:Percutaneous foraminal endoscopic BEIS technique has excellent short-term effect,less trauma and rapid recovery in the treatment of senile lumbar disc herniation.
