摘 要:目的:探讨希森美康XN-550测定尿红细胞平均体积(MCV)和红细胞体积分布宽度(RDW)检测对鉴别血尿来源的诊断价值。方法:选择2021年1月~2022年6月本院检验科接收的血尿样本90例为对象,根据镜检或(及)病理组织活检结果分组,肾小球来源组(n=47)和非肾小球来源组(n=43)。比较肾小球来源与非肾小球来源血尿的MCV、RDW水平,观察尿红细胞MCV和RDW指标单一检测与联合检测的结果,根据文献评价每种检测指标的血尿来源鉴别诊断效能。结果:肾小球来源组MCV水平(59.05±12.57)fL低于非肾小球来源组(96.43±10.19)fL,RDW水平(45.40±8.93)%高于非肾小球来源组(36.16±10.85)%,P<0.05。尿红细胞MCV和RDW检测鉴别诊断肾小球来源血尿的灵敏度95.74%、特异度93.02%和准确度94.44%高于尿红细胞MCV检测、RDW检测单一检测,P<0.05。结论:使用希森美康XN-550检测尿红细胞MCV和RDW提高鉴别诊断血尿来源的效能显著,具有较高的灵敏度与特异度。Objective:To investigate the diagnostic value of urine mean red blood cell volume(MCV)and red blood cell volume distribution width(RDW)measured by XN-550 in differentiating the origin of hematuria.Methods:A total of 90 hematuria samples received from our laboratory department from January 2021 to June 2022 were selected as subjects.According to the results of microscopic examination or(and)pathological biopsy,they were divided into glomerular origin group(n=47)and non-glomerular origin group(n=43).The levels of MCV and RDW in hematuria from glomerular and non-glomerular sources were compared,and the results of single and combined detection of MCV and RDW in urine red blood cells were observed.The differential diagnostic efficacy of each detection index was evaluated according to the literature.Results:The MCV level in glomerular origin group was(59.05±12.57)fL lower than that in non-glomerular origin group(96.43±10.19)fL,and the RDW level in glomerular origin group was(45.40±8.93)%higher than that in non-glomerular origin group(36.16±10.85)%,P<0.05.The sensitivity,specificity and accuracy of urinary red blood cell MCV and RDW in differential diagnosis of glomerular hematuria were 95.74%,93.02%and 94.44%,respectively,higher than those of urinary red blood cell MCV and RDW alone,P<0.05.Conclusion:The detection of MCV and RDW in urine red blood cells by XN-550 has significant efficacy in the differential diagnosis of hematuria,and has high sensitivity and specificity.
