摘　　要：目的:探讨床旁X线摄片对新生儿坏死性小肠结肠炎(NEC)的诊断价值。方法:回顾性分析57例经临床确诊NEC的患儿的床旁腹部X线征象和临床表现。结果:57例NEC患儿Ⅰ期15例、Ⅱ期22例、Ⅲ20例,均进行了1次或1次以上床旁X线摄片,X线影像示肠管扩张积气48例,肠管形态欠规则、肠间隙增宽39例,肠梗阻11例,肠壁积气21例,门静脉积气8例,腹腔游离气体18例。早产儿47例,极低体重儿(体重低于1500g)18例,行腹腔镜或剖腹探查并置管引流者29例,仅腹腔穿刺引流者7例。结论:床旁X线影像足以诊断NEC,并且摄片方便、高效,有助于随时评估新生儿腹部肠管情况。Objective: To explore the value of mobile X-ray radiography in diagnosis of neonatal necrotizing enterocolitis(NEC). Methods: Abdominal X-ray images and clinical features of 57 patients with NEC were analyzed retrospectively. Results: Among 57 patients with NEC, 15 cases in stage Ⅰ, 22 cases in stage Ⅱ and 20 cases in stage Ⅲ. All cases underwent one or more abdominal X-ray radiograph. 48 cases showed intestinal dilatation, 38 cases showed irregular shape of intestine and widening of intestinal space, 11 cases showed intestinal obstruction, 21 cases showed pneumatosis of intestinal wall, 8 cases showed pneumatosis of portal vein and 18 cases showed pneumoperitoneum. There were 47 premature infants, 18 very low birth weight infants(weight less than 1500g), 29 cases underwent laparoscopic or laparotomy exploration with catheter drainage, and 7 cases only underwent abdominal puncture drainage. Conclusion: Mobile X-ray image is sufficient to diagnose NEC, and the radiography is convenient and efficient, which is helpful to evaluate the abdominal intestinal tract of newborn at any time.
