超声造影对颈部淋巴结疾病诊断价值分析

摘　　要：目的:分析超声造影对颈部淋巴结疾病的诊断价值。方法:回顾性分析本院2019年1月~2020年5月诊治的52例疑似颈部淋巴结疾病患者作为本次试验研究对象,52例患者分别进行常规超声检查(对照组)和超声造影检查(观察组),以病理学诊断为金标准,比较两组诊断效果。结果:与对照组的良性诊断确诊率和恶性诊断确诊率相比,观察组均明显偏高(P<0.05);超声造影检查18例良性患者中结核性淋巴结占比33.33%,反应性增生淋巴结占比66.67%,33例恶性患者中转移性淋巴结占比69.70%,淋巴瘤占比30.30%。有23例为转移性淋巴结,占比为70.59%,造影图像为不均匀强化,表现为不均匀弥散状况,以周边向内部充填模式为主,缺少淋巴门样血流,且消退较快,可伴有不规则无增强区。结论:相对比于常规超声检查来说,给予颈部淋巴结疾病患者超声造影检查能够获取到比较准确、全面的诊断结果。Objective:To analyze the diagnostic value of contrast-enhanced ultrasound in cervical lymph node diseases.Methods:A retrospective analysis of our hospital from January 2019 to May 2020 in 52 cases of suspected cervical lymph node disease patients as the research object,52 cases of patients were given conventional ultrasound examination(control group)and contrast-enhanced ultrasound(observation group),pathological diagnosis as the gold standard,compared the diagnostic effect of the two groups.Results:Compared with the control group,the positive rate of benign diagnosis and the positive rate of malignant diagnosis in the observation group were significantly higher(P<0.05);Among 18 benign patients,33.33%were tuberculous lymph nodes,66.67%were reactive hyperplasia lymph nodes,69.70%were metastatic lymph nodes,and 30.30%were lymphoma.There were 23 cases of metastatic lymph nodes,accounting for 70.59%.The angiographic images showed uneven enhancement,showing uneven dispersion.The filling mode was mainly from the periphery to the interior,lacking lymphatic hilum like blood flow,and rapidly subsiding,which may be accompanied by irregular areas without enhancement.Conclusion:Compared with conventional ultrasound,CEUS can obtain more accurate and comprehensive diagnosis results for patients with cervical lymph node disease.
