摘 要:目的观察调理中焦气机法对脓毒症大鼠肠道屏障损伤及炎症因子的影响。方法SD大鼠采用盲肠结扎法建立脓毒症大鼠模型,随机分为模型组,中药高、中、低剂量组,每组各8只,另取SD大鼠8只仅做开腹处理作为假手术组,各组分别予对应药物灌胃。HE染色法观察大鼠组织病理学改变,酶联免疫吸附法(ELISA)检测血清内毒素、D-乳酸、二胺氧化酶及血清白细胞介素-1β(IL-1β)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平,免疫组化法(IHC)检测大鼠肠组织中IL-1β、IL-6、TNF-α蛋白表达水平。结果与假手术组比较,模型组黏膜层可见大面积上皮细胞脱落,固有层裸露,局部可见溃疡,伴有较多淋巴细胞浸润,局部可见静脉血管淤血扩张。与模型组比较,中药各干预组黏膜层损伤减轻,以高剂量组最轻,损伤评分差异有统计学意义(P<0.01)。与假手术组相比,模型组大鼠血清内毒素、D-乳酸、二胺氧化酶水平明显升高(P<0.01),血清IL-1β、IL-6、TNF-α水平均显著上升(P<0.01),肠组织IL-1β、IL-6、TNF-α蛋白表达水平升高(P<0.05)。与模型组相比,中药各剂量组大鼠血清内毒素、D-乳酸、二胺氧化酶水平有不同程度的降低(P<0.05或P<0.01),血清IL-1β、IL-6、TNF-α水平明显降低(P<0.01),肠组织IL-1β、IL-6、TNF-α蛋白表达水平不同程度降低(P<0.05)。结论调理中焦气机法可减轻脓毒症大鼠肠道屏障损伤,其机制可能与其抑制炎症因子IL-1β、IL-6、TNF-α释放有关。Objective:To investigate the effect of conditioning Zhongjiao Qi mechanism on intestinal barrier damage and inflammatory factors in sepsis rats. Methods:The SD rats were established by cecal ligation(CLP)to establish a rat model of sepsis,and the successfully established rats were randomly divided into CLP group,highdose,medium-dose,and low-dose Chinese medicine groups,with 8 rats in each group,and normal SD rats were selected. 8 rats were only treated by laparotomy as the sham group,and each group was given the corresponding drugs by gavage. After 24 hours of administration,the test specimens were collected,and the histopathological changes of the rats were observed by HE staining;serum endotoxin,D-lactic acid,diamine oxidase,and serum IL-1β,IL-6,TNF-α levels were detected by enzyme-linked immunosorbent assay(ELISA);immunohistochemical method(IHC)was used to detect the IL-1β,IL-6,TNF-α protein expression levels in rat intestinal tissue.Results:Compared with the sham group,HE staining showed that the mucosal layer of the CLP group showed a large area of epithelial cells shedding,the lamina propria was exposed,and local ulcers were seen,accompanied by more lymphocyte infiltration,and local venous blood vessel congestion and expansion. Compared with the CLP group,the mucosal damage in each intervention group of traditional Chinese medicine was alleviated,and the highdose group was the least,and the damage score was statistically significant(P < 0.01). Compared with the sham group,the levels of serum endotoxin,D-lactic acid,and diamine oxidase in the CLP group were significantly increased(P < 0.01),and the levels of serum IL-1β,IL-6,and TNF-α were significantly increased(P < 0.01),the protein expression levels of IL-1β,IL-6 and TNF-α in intestinal tissue increased(P < 0.05).
