自拟活血败脓散联合超声引导下穿刺治疗盆腔脓肿的疗效观察

摘　　要：目的观察自拟活血败脓散联合超声引导下穿刺治疗盆腔脓肿的疗效以及对复发的预防作用。方法80例盆腔脓肿患者按随机数字表法分为两组,各40例。对照组采用超声引导下穿刺引流,观察组采用超声引导下穿刺引流联合自拟活血败脓散治疗,疗程均为1个月。比较两组疗效、发热消退时间、腹痛消退时间、白细胞计数(WBC)恢复正常时间,细胞免疫和炎症因子,不良反应以及复发情况。结果观察组总有效率为87.50%,高于对照组的65.00%(P<0.05),观察组发热消退时间、腹痛消退时间、白细胞恢复正常时间短于对照组(P<0.05)。两组治疗后WBC、血沉(ESR)、CD8+细胞占比、血清C反应蛋白(CRP)、白细胞介素-6(IL-6)水平均较治疗前降低(P<0.05),外周血CD3+细胞占比、CD4+细胞占比、CD4+/CD8+均较治疗前增高(P<0.05),观察组治疗后WBC、ESR、CD8+细胞占比、血清CRP、IL-6水平低于对照组(P<0.05),外周血CD3+细胞占比、CD4+细胞占比、CD4+/CD8+高于对照组(P<0.05)。失访3例,观察组复发率低于对照组(P<0.05)。结论超声引导下穿刺引流联合自拟活血败脓散治疗可更有效抑制炎症反应,提高细胞免疫功能,促使临床症状改善,提高临床治疗效果,预防复发。Objective:To investigate the efficacy of ultrasound-guided puncture combined with self-designed Huoxie Bainong Powder in the treatment of pelvic abscess and the prevention of recurrence.Methods:A total of80 patients with pelvic abscess admitted to our hospital were selected and divided into two groups by random number table method,40 cases in each group.The control group was treated with ultrasound-guided puncture and drainage,and the observation group was treated with ultrasound-guided puncture and drainage combined with selfdesigned Huoxue Bainong Powder.The efficacy,fever subsidence,abdominal pain resolution time,white blood cell(WBC)recovery time,cellular immunity and inflammatory factors,adverse reactions and recurrence were compared between the two groups.Results:The effective rate of treatment in the observation group was higher than that in the control group(P<0.05),and the time for fever subsidence,abdominal pain subsidence,and WBC recovery time in the observation group were lower than those in the control group(P<0.05).After treatment,the proportions of WBC,ESR,CD8+cells,serum CRP,and IL-6 in the two groups were lower than those before treatment(P<0.05),and these levels in the observation group were lower than those in the control group(P<0.05).The proportion of CD3+cells in peripheral blood,the proportion of CD4+cells,and CD4+/CD8+were all higher than those before treatment(P<0.05),and these levels in the observation group were higher than those in the control group(P<0.05).Three cases were lost during follow-up,and the recurrence rate in the observation group was lower than that in the control group(P<0.05).Conclusion:Ultrasound-guided puncture and drainage combined with self-designed Huoxue Bainong Powder can effectively inhibit inflammatory response,improve cellular immune function,improve clinical symptoms,improve clinical treatment effect and prevent recurrence.
