扶正泄浊保肾汤治疗对急性间质性肾炎患者肾功能及血液流变学指标的影响

摘　　要：目的观察扶正泄浊保肾汤治疗对急性间质性肾炎(AIN)患者肾功能及血液流变学指标的影响。方法将70例AIN患者按照随机数字表法分为观察组与对照组各35例。对照组患者予西医常规治疗,观察组患者在对照组治疗基础上采用扶正泄浊保肾汤治疗。观察两组中医证候疗效、肾功能、血液流变学指标和尿液检查相关指标的差异性。结果观察组中医证候总有效率为85.71%,明显高于对照组的62.86%(P<0.05)。治疗后两组尿素氮(BUN)、肌酐(Scr)、β2微球蛋白(β2-MG)水平较治疗前均下降(P<0.05),观察组下降程度更明显(P<0.05)。治疗后两组低切全血比黏度、血浆黏度和血细胞比容水平较治疗前均下降(P<0.05),观察组下降程度更明显(P<0.05)。治疗后两组肾损伤因子(KIM-1)、中性粒细胞明胶酶相关脂质运载蛋白(NGAL)、24 h尿蛋白定量较治疗前均下降,肾小球滤过率(eGFR)水平较治疗前均上升(P<0.05),观察组改善程度更明显(P<0.05)。结论将扶正泄浊保肾汤应用于AIN的治疗中,可有效提高中医证候疗效,改善肾功能和血液流变学指标水平,减轻肾损伤程度,缓解病情严重程度。Objective:To investigate the effect of Fuzheng Xiezhuo Baoshen Decoction on renal function and hemorheology in patients with acute interstitial nephritis(AIN).Methods:70 patients with AIN were divided into an observation group and a control group according to the random number table method,with 35 cases in each group.The control group was treated with conventional western medicine,and the observation group was treated with Fuzheng Xiezhuo Baoshen Decoction on the basis of the treatment in the control group.The differences in the efficacy of TCM syndromes,renal function,hemorheology,and urine test-related indicators between the two groups were observed.Results:The total effective rate of TCM syndrome in the observation group was 85.71%,which was significantly higher than 62.86%in the control group(P<0.05).After treatment,the levels of BUN,Scr andβ2-MG in the two groups were lower than those before treatment(P<0.05),and the decrease in the observation group was more obvious(P<0.05).After treatment,the low shear whole blood viscosity,plasma viscosity and hematocrit levels in the two groups were lower than those before treatment(P<0.05),and the decrease in the observation group was more obvious(P<0.05).After treatment,the levels of KIM-1,NGAL and 24 h urinary protein in the two groups were decreased,the levels of eGFR were increased compared with those before treatment(P<0.05),and the improvement in the observation group was more obvious(P<0.05).Conclusion:The application of Fuzheng Xiezhuo Baoshen Decoction in the treatment of AIN can effectively enhance the efficacy of TCM syndromes,improve renal function and hemorheology index levels,reduce the degree of renal injury,and alleviate the severity of the disease.
