摘 要:目的设计便携式体外膜肺氧合(extracorporeal membrane oxygenation,ECMO)急救箱并了解其应用效果。方法设计便携式ECMO急救箱并进行管理,包括归纳整理ECMO相关物品列出清单,划分6个类别分别置于可透视的袋中并标识置于急救箱中,采用加锁管理和按照“定人、定期、定质、定量”进行检查。采用方便抽样方法,选取2020年1月至12月需实施ECMO抢救的急危重症患者18例设为对照组,运用传统急救包进行急救;选取2021年1月至4月需实施ECMO抢救的急危重症患者18例患者设为观察组,运用便携式ECMO急救箱进行急救。比较两组备物时间及物品完好率的差异。结果观察组备物时间(5.0±0.3)min较对照组(35.2±18.0)min短,差异有统计学意义(t=-56.585,P<0.001)。观察组物品完好率94.4%高于对照组44.4%,差异有统计学意义(χ2=10.166,P<0.05)。结论便携式ECMO急救箱保证了物品完好性并缩短了备物时间,并方便医护人员在急救时快速找到物品,从而提高抢救成功率,同时ECMO急救箱取材方便、制作简单、便于携带,值得临床推广应用。Objective Aportable emergency kit for extracorporeal membrane oxygenation(ECMO)was designed and investigated. Methods A portable emergency kit for ECMO was designed with a list of ECMO items and it was applied clinically.According to 6categories of the items, the listed ECMO items were sorted, labelled and placed in a transparent plastic bag. The kit was managed with a lock and scheduled inspections carried out in accordance with the assigned person, scheduled checks to ensure therequired quality and quantity. The convenient sampling method was used to select 18 critical patients who received ECMO treatment between January and December 2020 as control group, and another 18 critical patients with the same conditions between January and April 2021 were assigned in the observation group. The control group was managed with the ordinary first-aid kits, and the designed portable kits for ECMO were applied to the patients in observation group. The two groups were compared in terms of the time for fetching emergency supplies and the intact rate of items. Results The preparation time in the observation group was significantly shorter than that in the control group[(5.0 ± 0.3) min vs. 35.2 ± 18.0) min, t=-56.585, P<0.001]. The intact rate of items in the observation group was significantly higher than that of the control group(94.4% vs.44.4%, 10.166, P<0.01). Conclusions The portable emergency kit for ECMO ensures a great reduction in the time of preparation and integrity of the requireditems. It facilitates a swift item fetching in an emergency, thus improves the success rate in patient rescue. Moreover, the portable kit for ECMO features high efficiency in itemmanagement, simple in design,easy to make and easy to carry. It is an ideal too lfor clinical application.
