“形神共调”针刺法治疗脑卒中后肢体麻木病人感觉障碍的临床疗效及脑电作用机制研究

摘　　要：目的探讨“形神共调”针刺法对脑卒中后肢体麻木病人感觉障碍的临床疗效及脑电作用机制研究。方法于2020年10月—2021年7月在北京中医药大学东直门医院招募脑卒中发病>6个月伴有肢体麻木的病人24例进行“形神共调”针刺法治疗,并在治疗前后采用数字模拟评级法(VAS)评估病人肢体麻木感变化;采用诺丁汉感觉评价量表(NSA)评估患侧肢体感觉障碍,同时采用脑电图检测观察治疗前后脑电节律平均功率及其与VAS评分、NSA评分的相关性。结果治疗4周后,病人VAS评分较治疗前明显降低(P<0.01);NSA评分较治疗前明显升高(P<0.01);β波平均功率在FP1、FP2、AF3、T7导联处较治疗前明显提高(P<0.05),且主要集中在额叶;β波幅值平方相干在OZ与P1,PO6与P6、P4、P2、CP4,POZ与P2、P3,FT8与F8,F6与F8等导联之间增强。结论形神共调针刺法能有效改善脑卒中麻木病人肢体麻木症状、恢复感觉功能以达到“调形”的作用,且可通过调节与感觉功能相关的脑电活动起到“调神”并最终达到“形神共调”的疗效。Objective To investigate the clinical efficacy and mechanism of electroencephalogram(EEG)action of"Co-regulation of the body and the spirit"acupuncture on sensory disorders in patients with limb numbness after stroke.Methods From October 2020 to July 2021,24 stroke patients with limb numbness more than 6 months after onset were recruited and treated with"Co-regulation of the body and the spirit"acupuncture.Visual Analogue Scal(VAS)was used to evaluate the changes of limb numbness before and after treatment.Nottingham Sensory Rating Scal(NSA)was used to evaluate the sensory impairment of the affected limb.At the same time,EEG was used to observe the average power of EEG rhythm before and after treatment and its correlation between VAS score and NSA score.Results After 4 weeks of treatment,the VAS score of patients was significantly lower than that before treatment(P<0.01).The NSA score was significantly higher than that before treatment(P<0.01).The averageβrhythm in FP1,FP2,AF3,and T7 leads was significantly higher than that before treatment(P<0.05),and which was mainly concentrated in the frontal lobe.The Amplitude Square coherence ofβrhythm in OZ and P1;PO6 and P6,P4,P2,CP4;POZ and P2,P3;FT8 and F8;F6 and F8 etc.were enhanced.Conclusion"Co-regulation of the body and the spirit"acupuncture can effectively improve limb numbness symptoms of stroke patients with numbness and restore sensory function to achieve the effect of"regulation of body",and"regulation of spirit"by regulating the brain electrical activity related to sensory function.
