柳州市含麻疹成分疫苗疑似预防接种异常反应监测分析

摘　　要：目的对柳州市含麻疹成分疫苗(MCV)免疫程序调整前后的疑似预防接种异常反应(AEFI)报告率及流行特征进行分析。方法通过中国免疫规划信息管理系统AEFI模块信息管理系统和中国疾病预防控制信息系统免疫规划AEFI模块监测管理系统收集柳州市2019年6月1日—2021年5月31日MCV AEFI监测资料,比较MCV免疫程序调整前后AEFI报告率及流行特征。结果柳州市MCV AEFI报告率34.76/10万,其中一般反应报告率13.09/10万,异常反应报告率21.67/10万。AEFI报告的临床症状最多的是发热和过敏性皮疹(报告率分别为20.31/10万、15.35/10万)。免疫程序调整后,MCV AEFI报告率变化无统计学意义(P>0.05);麻腮风疫苗(MMR)AEFI报告率和过敏性皮疹报告率均出现上升(P<0.05)。结论免疫程序调整后,应用MMR替代麻风疫苗(MR)并没有显著增加AEFI报告率,且未造成原有MCV AEFI流行特征的变化。Objective To analyze the reporting rate and epidemiological characteristics of adverse events following immunization(AEFI)before and after the adjustment of immunization program for measles containing vaccine(MCV)in Liuzhou City.Methods MCV AEFI surveillance data of Liuzhou City from June 1,2019 to May 31,2021 was collected through AEFI information management sub-system of China Immunization Program Information Management System and monitoring management sub-system for immunization program AEFI of China Information System for Disease Prevention and Control,and the AEFI reporting rates and epidemiological characteristics before and after the adjustment of MCV immunization program were compared.Results The reporting rate of MCV AEFI in Liuzhou was 34.76/100000,including 13.09/100000 for general reactions and 21.67/100000 for AEFI.The most frequently reported clinical symptoms in AEFI were fever and allergic rash with the reporting rates 20.31/100000 and 15.35/100000 respectively.After the adjustment of immunization program,there was no statistically significant difference in the changes of MCV AEFI reporting rate(P>0.05),and the reporting rates of AEFI and allergic rash in measles-mumps-rubella(MMR)vaccine were all increased(P<0.05).Conclusion After the adjustment of immunization program,the application of MMR instead of MR does not significantly increase the AEFI reporting rate and does not cause changes in the epidemiological characteristics of MCV AEFI.
