小剂量异维A酸联合微针导入类人胶原蛋白治疗痤疮后瘢痕疗效观察

摘　　要：目的:观察口服小剂量异维A酸的同时予以微针导入类人胶原蛋白,治疗痤疮后瘢痕的疗效。方法:选取14例正在服用小剂量异维A酸(0.2~0.4 mg/kg,每日1次)以痤疮瘢痕为主的患者。随机将其面部两侧分为治疗组和对照组,治疗组使用0.5~1.0 mm微针导入类人胶原蛋白进行治疗,每2周治疗1次,共治疗4次,术后予以保湿防晒护理。对照组仅予以保湿防晒护理。观察并比较两组的疗效、VISIA各项指标、疼痛评分、术后不良反应。结果:治疗组有效率为78.57%,对照组有效率为28.57%,治疗组有效率高于对照组,差异具有统计学意义(χ^(2)=5.169,P=0.023)。治疗后,治疗组紫外线色斑较对照组改善,差异有统计学意义(t=-2.420,P=0.022)。治疗组的斑点(t=3.124,P=0.007)、皱纹(t=-4.448,P=0.000)、毛孔(t=3.694,P=0.002)、紫外线色斑(t=2.728,P=0.016)、紫质(t=3.768,P=0.002)等指标较治疗前均有好转,差异有统计学意义(P<0.05)。14例患者对疼痛的视觉模拟评分为2~4分,均为轻度,可耐受。14例患者治疗后均未出现明显的不良反应。结论:小剂量异维A酸联合微针导入类人胶原蛋白治疗痤疮后瘢痕安全有效,为临床上寻常痤疮患者提供了一种安全有效的治疗方法,值得临床应用和推广。Objective To explore the clinical efficacy and safety of of low-dose oral isotretinoin combined with human-like collagen after microneedle therapy in patients with acne scars.Methods 14 patients who were treated with low-dose isotretinoin were recruited.Different side of the patients’face was randomly divided into the control group and the treatment group.The treatment group was treated with human-like collagen after 0.5~1.0 mm microneedle therapy.The two groups were given the same sun protection and moisturizing care.The efficacy,VISIA indicators,pain scores,and postoperative adverse reactions of the two groups were observed and compared.Results The effective rate in the treatment group was 78.57%,that of the control group was 28.57%,and the difference was statistically significant(χ^(2)=5.169,P=0.023).After treatment,Ultraviolet spots in the treatment group were better than those in the control group,and the difference was statistically significant(t=-2.42,P=0.022).In the treatment group,spots(t=3.124,P=0.007),wrinkles(t=-4.448,P=0.000),pores(t=3.694,P=0.002),ultraviolet spots(t=2.728,P=0.016)and porphyrin(t=3.768,P=0.002)were improved,and the differences were statistically significant(P<0.05).The visual simulation scores of pain in 14 patients were 2 to 4 points,which were mild and tolerated.None of the 14 patients showed significant adverse reactions after treatment.Conclusion Low-dose isotretinoin combined with human-like collagen after microneedle therapy in acne scars is effective,with no serious adverse reactions,which provides a safe and effective treatment for acne scars.It is worthy of clinical application and promotion.
