单叶产钳应用于再次剖宫产产妇分娩的临床价值分析

摘　　要：目的分析单叶产钳用于再次剖宫产产妇分娩的临床价值,为临床提供参考。方法选取2020年1月至12月揭阳市榕城区妇幼保健计划生育服务中心收治的120例再次剖宫产产妇为研究对象进行回顾性分析,根据不同的助产方式分为实验组(60例,采取单叶产钳助产)和对照组(60例,采取常规助产)。比较两组产妇围术期指标、胎儿胎头娩出一次成功率、胎头娩出时间、新生儿评分量表(Apgar)评分、产妇及新生儿并发症发生情况。结果实验组产妇手术时间和恶露干净时间短于对照组,术中出血量和术后24 h出血量少于对照组(P<0.05)。实验组胎儿胎头娩出一次成功率和Apgar评分高于对照组,胎头娩出时间短于对照组(P<0.05)。实验组产妇并发症发生率低于对照组(P<0.05)。实验组新生儿并发症发生率低于对照组(P<0.05)。结论单叶产钳用于再次剖宫产术中可缩短胎头娩出时间和手术时间,提高胎儿胎头娩出一次成功率,减少产妇术中术后出血量及产妇与新生儿并发症。Objective To analyze the clinical value of single-leaf forceps for parturient with second cesarean section,and to provide reference for clinical practice.Methods According to different midwifery methods,120 parturients who underwent second cesarean section in Rongcheng District Maternal and Child Health and Family Planning Service Center of Jieyang City from January 2020 to December 2020 were divided into experimental group(treated with single-leaf forceps)with 60 cases and control group(treated with conventional midwifery)with 60 cases.The perioperative indicators,the first successful rate of fetal delivery,the time of fetal head delivery,Apgar score,and the incidence of maternal and neonatal complications were compared between the two groups.Results The operation time and lochia cleaning time of the experimental group were shorter than those of the control group,and the intraoperative blood loss and 24 h postoperative blood loss were less than those of the control group(P<0.05).The success rate and Apgar score of fetal head delivery in the experimental group were higher than those in the control group,and the fetal head delivery time in the experimental group were shorter than that in the control group(P<0.05).The incidence of maternal complications in the experimental group were lower than that in the control group(P<0.05).The incidence of neonatal complications in the experimental group were lower than that in the control group(P<0.05).Conclusion The use of single-leaf forceps in cesarean section can shorten the time of fetal head delivery and operation time,improve the success rate of the first delivery of the fetus,reduce intraoperative and postoperative blood loss,maternal and neonatal complications.
