摘　　要：目的观察呼吸振动正压排痰装置Acapella Duet联合雾化吸入乙酰半胱氨酸治疗老年支气管扩张症急性加重期患者的临床效果。方法选择2020年1月—2021年12月于上海市浦东新区浦南医院住院治疗的老年支扩症急性加重期患者90例,根据数字表法随机分为对照组、试验A组和试验B组,每组30例。对照组给予左氧氟沙星注射液抗感染和静脉氨溴索化痰治疗;试验A组在与对照组相同治疗基础上加用喷雾罐雾化吸入乙酰半胱氨酸;试验B组应用Acapella Duet雾化吸入乙酰半胱氨酸。3组疗程均为10 d。比较3组的临床症状评分(VAS评分和咳痰量评分)、mMRC分级、炎症指标值、住院时间、抗生素使用时间和住院费用的差异。结果与同组治疗前及对照组治疗后比较,试验A组和试验B组治疗后VAS评分、咳痰量评分、mMRC分级和炎症指标值均明显降低,住院时间、抗生素使用时间和住院费用明显减少,试验B组降幅比试验A组更大,差异均有统计学意义(P<0.05)。结论Acapella Duet雾化吸入乙酰半胱氨酸可显著改善老年支气管扩张症急性加重期患者的临床症状,减轻炎症反应,减少住院时间、抗生素使用时间和住院费用。Objective To observe the clinical effects of respiratory vibration positive pressure sputum drainage device Acapella Duet combined with aerosol inhalation of acetylcysteine in the treatment of elderly patients with acute exacerbation of bronchiectasis.Methods A total of 90 elderly patients with acute exacerbation of bronchiectasis hospitalized in Shanghai Punan Hospital in Pudong New District from January 2020 to December 2021 were selected and randomly divided into control group,experimental group A and experimental group B according to the number table method,with 30 cases in each group.The control group was treated with levofloxacin injection for anti-infection andintravenous ambroxol for expectoration.On the basis of the same treatment as that of the control group,the experimental group A added the aerosol inhalation of acetylcysteine with spray cans and experimental group B did aerosol inhalation of acetylcysteine by Acapella Duet.The treatment course of the three groups was 10 days.The clinical symptom scores(VAS score and expectoration volume score),mMRC grade,inflammation index values,hospitalization time,duration of antibiotic use and hospitalization cost were compared among the three groups.Results Compared with the same group before treatment and the control group after treatment,the VAS score,expectoration volume score,mMRC grade and inflammatory index value of the experimental group A and experimental group B after treatment were significantly reduced.The hospitalization time,duration of antibiotic use and hospitalization cost of theexperimental group A and experimental group B were significantly shorter or less than those of the control group.The reduction range of the experimental group B was greater than that of the experimental group A.All of above differences were statistically significant(P<0.05).Conclusion The aerosol inhalation of acetylcysteine by Acapella Duet can significantly improve the clinical symptoms of elderly patients with acute exacerbation of bronchiectasis,reduce inflammatory re
